丹参多酚酸盐联合尼可地尔治疗老年冠心病不稳定型心绞痛的效果及对血栓前状态、炎症反应的影响

摘　　要：目的探讨丹参多酚酸盐联合尼可地尔治疗老年冠心病不稳定型心绞痛(UAP)的效果及对血栓前状态、炎症反应的影响。方法选取2018年9月至2019年10月我院接收的65例老年冠心病UAP患者作为研究对象,按照治疗方案将其分为对照组(32例)与观察组(33例)。对照组给予常规对症治疗+尼可地尔,观察组在对照组基础上给予丹参多酚酸盐。比较两组患者的疗效、血栓前状态指标、炎症反应指标及血清趋化因子(CX3CL1)、血清脂肪因子(Vaspin)表达水平。结果观察组的治疗总有效率高于对照组(P<0.05)。治疗后,两组的FIB、D-D水平、AA诱导血小板聚集值、ADP诱导血小板聚集值均降低,且观察组低于对照组(P<0.05)。治疗后,两组的IL-4、IFN-γ、PCT水平均降低,且观察组低于对照组(P<0.05)。治疗后,两组的CX3CL1表达水平均降低,Vaspin表达水平均升高,且观察组优于对照组(P<0.05)。结论丹参多酚酸盐联合尼可地尔治疗老年冠心病UAP的效果显著,可有效改善患者的血栓前状态,还能控制机体炎症反应。Objective To explore the effect of salvianolate combined with nicorandil in the treatment of unstable angina pectoris(UAP)of elderly coronary heart disease and its influence on prethrombotic state and inflammatory response.Methods A total of 65 elderly patients with UAP of coronary heart disease received in our hospital from September 2018 to October 2019 were selected as the research objects.The patients were divided into control group(32 cases)and observation group(33 cases)according to the treatment scheme.The control group was given routine symptomatic treatment+nicorandil,and the observation group was given salvianolate on the basis of the control group.The curative effect,prethrombotic state indexes,inflammatory response indexes and the expression levels of serum chemokine(CX3CL1),serum adipokine(Vaspin)were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,FIB,D-D levels,AA induced platelet aggregation and ADP induced platelet aggregation in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the levels of IL-4,IFN-γand PCT in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,the expression levels of CX3CL1 decreased and Vaspin increased in the two groups,and those in the observation group were better than the control group(P<0.05).Conclusion Salvianolate combined with nicorandil in the treatment of elderly patients with UAP of coronary heart disease has a significant effect,it can effectively improve the prethrombotic state of patients and control the inflammatory response of the body.
