Value analysis of four-dimensional contrast-enhanced ultrasound combined with quantitative detection of uterine pressure in the prediction of tubal patency

摘  要:目的比较四维超声造影联合宫腔压力定量检测与X线输卵管造影对输卵管通畅性的预估价值。方法分别采用四维超声造影联合压力测定、X线子宫输卵管造影评估260例不孕患者的输卵管通畅性,以腹腔镜通染液实验结果为金标准,比较两者的检测准确性。结果 X线子宫输卵管造影结果显示,260例患者中有108例输卵管通畅;四维超声造影结果显示,260例患者中有81例输卵管通畅。输卵管不通畅的压力值高于输卵管完全通畅,X线子宫输卵管造影、宫腔压力测量的输卵管不通畅检出数与腹腔镜通染液实验比较,差异均有统计学意义(P<0.05);四维超声造影联合压力测定的输卵管不通畅检出数与腹腔镜通染液实验差异无统计学意义(P>0.05)。四维超声造影联合压力测定、X线输卵管造影检测后,所有患者均未发生严重并发症。结论四维超声造影及宫腔压力定量检测预估输卵管通畅性准确性高、操作简单、安全快速,宫腔压力值大小直接反映输卵管的通畅性。Objective To compare the value of four-dimensional contrast-enhanced ultrasound combined with quantitative detection of intrauterine pressure and X-ray salpingography in the prediction of tubal patency.Methods Four-dimensional contrast-enhanced ultrasound combined with pressure detection and X-ray hysterosalpingography were used to evaluate the tubal patency of 260 infertile patients respectively.Taking the results of laparoscopic staining solution experiment as the gold standard,the detection accuracy of the two methods was compared.Results X-ray hysterosalpingography results showed that 108 out of 260 patients had unobstructed fallopian tubes.The results of four-dimensional contrast-enhanced ultrasound showed that 81 out of 260 patients had unobstructed fallopian tubes.The pressure value of blocked fallopian tube was higher than that of completely unblocked fallopian tube.Compared with the number of blocked fallopian tubes detected by the laparoscopic staining solution experiment,that detected by X-ray hysterosalpingography or by intrauterine pressure measurement presented the difference that were statistically significant(P<0.05).There was no statistically significant difference in the number of blocked fallopian tubes detected by four-dimensional contrast-enhanced ultrasound combined with pressure measurement and by the laparoscopic staining solution experiment(P>0.05).After the detection of four-dimensional contrast-enhanced ultrasound combined with pressure measurement and X-ray salpingography,none of the patients had serious complications.Conclusions Four-dimensional contrast-enhanced ultrasound and quantitative detection of uterine cavity pressure predicted the tubal patency with high accuracy and simple operation,safe and fast.The value of the uterine pressure directly reflected the patency of the fallopian tube.
