摘  要:目的:采用Fisher判别函数鉴别诊断模型评价超声弹性成像对乳腺结节的诊断价值。方法:回顾性分析145例患者(共199枚)乳腺结节的超声图像特点,以术后病理结果为金标准分良、恶性两组,对两组间存在统计学差异的特征参数进行Fisher判别分析。结果:共纳入10个具有统计学差异的参数,包括:乳腺结节血流分级,结节形态、方位、内部回声、后方回声特征、钙化、结节边缘、YmM(肿块杨氏模量值), YmT(脂肪组织杨氏模量)及YmM/YmT比值。回代性检验评价建立的判别函数鉴别诊断模型对乳腺结节诊断的特异性、敏感性及诊断准确率分别为97.89%、87.72%及94.97%;刀切法评价诊断符合率为91.3%。结论:应用Fisher判别分析法建立的诊断模型有助于克服主观影响,为乳腺结节良恶性鉴别提供一定参考价值,具有较好的临床应用前景。Objective:To evaluate the diagnostic value of ultrasound elastography for breast nodules by using the Fisher discriminant function differential diagnosis model.Methods:The ultrasound image characteristics of 199 breast nodules in 145 patients were retrospectively analyzed.According to postoperative pathological results, the breast nodules were divided into benign and malignant groups.The Fisher discriminant analysis was performed on the characteristic ultrasound parameters with statistical significances between the two groups.Results:A total of 10 characteristic parameters with statistical significances were included, including blood flow grade, shape, orientation, internal echo, posterior echo characteristics, calcification, margin, YmM(Young’s modulus in mass),YmT(Young’s modulus in adipose tissue),and YmM/YmT ratio.The specificity, sensitivity, and diagnostic accuracy of the established discriminant function differential diagnosis model for breast nodules were 97.89%,87.72%,and 94.97%,respectively, and the diagnostic accuracy of the knife-cut method was 91.3%.Conclusion:The diagnostic model established by Fisher discriminant analysis can help to overcome the subjective influence, provide a certain reference value for the differentiation of benign and malignant breast nodules, and has a good clinical application prospect.
