摘 要:目的探讨脐带血白介素(IL)-13水平与早期婴儿湿疹的关系。方法选取2018年10月至2019年6月在中国人民解放军空军特色医学中心妇产科出生的148例足月健康新生儿为研究对象。采集新生儿脐带血,应用酶联免疫吸附法测定脐带血中IL-13的含量,登记新生儿出生的信息及家族过敏史等基本资料。随访42天婴儿并记录患有湿疹的情况,根据婴儿是否患有湿疹将其分为湿疹组87例和无湿疹组61例,对记录数据进行统计分析。结果在148例婴儿中,有87例患湿疹,患病率为58.8%。湿疹组脐带血IL-13水平明显高于无湿疹组(Z=-2.217,P=0.027)。湿疹组婴儿有家族过敏史的湿疹发生率明显高于无湿疹组(χ^(2)=4.452,P=0.035),湿疹组以混合喂养的婴儿湿疹发生率明显高于无湿疹组(χ^(2)=4.643,P=0.031),湿疹组母亲哺乳期进食海鲜类食物的婴儿湿疹发生率明显高于无湿疹组(χ^(2)=8.779,P=0.030)。经Logistic回归分析结果显示,有家族过敏史、母亲哺乳期有进食海鲜类食物、混合喂养、脐带血IL-13高水平是影响湿疹发生的危险因素(Waldχ^(2)值分别为11.439、8.526、4.572、5.411,P<0.05)。根据脐带血IL-13的受试者工作特征(ROC)曲线可知,曲线下面积(AUC)为0.712,95%CI:0.624~0.800,灵敏度为90.8%,特异度为49.2%。结论脐带血IL-13与婴儿湿疹的发生密切相关,是影响42天婴儿患有湿疹的重要因素。脐带血IL-13水平对早期婴儿湿疹的发生有一定预测意义。Objective To investigate the relationship between umbilical cord blood interleukin(IL)-13 level and early infantile eczema.Methods From October 2018 to June 2019,148 healthy full-term newborns, who were born in the department of obstetrics and gynecology of Air Force Medical Center of PLA,were selected as the research subjects.The umbilical cord blood of newborns was collected, and the content of IL-13 in the umbilical cord blood was determined by enzyme-linked immunosorbent assay.The basic information including birth information and family allergy history of newborns were registered.The infants were followed up for 42 days and their eczema condition was recorded.The infants were divided into the eczema group(n=87) and the non-eczema group(n=61) according to whether they had eczema or not.The recorded data was analyzed.Results Of the 148 infants, 87 cases developed eczema with the prevalence of 58.8%.The level of IL-13 in umbilical cord blood in the eczema group was significantly higher than that in the non-eczema group(Z=-2.217,P=0.027).The incidence of eczema in infants with family history of allergy in the eczema group was significantly higher than that in the non-eczema group(χ^(2)=4.452,P=0.035).The incidence of eczema in infants with mixed feeding in the eczema group was significantly higher than that in the non-eczema group(χ^(2)=4.643,P=0.031).The incidence of eczema in infants whose mother mothers had seafood food during lactation in the eczema group was significantly higher than that in the non-eczema group(χ^(2)=8.779,P=0.030).Logistic regression analysis results showed that with family history of allergy, mother′s consumption of seafood during lactation, mixed feeding and high level of umbilical cord blood IL-13 were the risk factors affecting the incidence of eczema, and the differences were statistically significant(Wald χ^(2)=11.439,8.526,4.572 and 5.411,respectively, P<0.05).According to the ROC curve of umbilical cord blood IL-13,the area under the receiver operating characteristic curve(AU
