摘  要:目的:探讨复方黄柏液涂剂外用对深Ⅱ度烧伤创面组织愈合及血管化的影响。方法:选择2018年3月-2019年12月承德医学院附属医院烧伤整形科确诊的深Ⅱ度烧伤患者60例,采用随机数字表法分为两组,两组均采用标准化创面处理,在此基础上,对照组(n=30)烧伤创面外用碘伏溶液,每日换药1次,治疗组(n=30)烧伤创面外用复方黄柏液涂剂,每日换药1次。两组于伤后7 d、14 d、21 d分别抽取静脉血及留取创面组织标本,按照酶联免疫吸附法测定血清及创面组织中血管内皮生长因子(VEGF)、成纤维细胞生长因子-7(FGF-7)水平,观察各组创面伤后第0天及第14天病理学改变,计算伤后14 d、21 d创面愈合率。结果:伤后7 d、14 d治疗组血清VEGF、FGF-7水平显著高于对照组,差异有统计学意义(P<0.05),伤后21 d两组血清VEGF、FGF-7水平对比,差异无统计学意义(P>0.05)。伤后7 d、14 d、21 d,治疗组创面组织VEGF、FGF-7水平显著高于对照组(P<0.05)。对照组、治疗组在伤后第7天出现上皮化,光镜下治疗组较对照组伤后第14天组织水肿减轻,炎症细胞浸润减少。伤后14 d、21 d,治疗组创面愈合率显著高于对照组(P<0.05)。结论:复方黄柏液涂剂外用可能通过促进部分生长因子的产生和释放,进而促进烧伤创面再血管化及创面愈合,值得临床推广应用。Objective To observe the effect of Compound Cortex Phellodendri Fluid of certain concentration as treatment on wound surface healing and neovascularization for skin with deep Ⅱ degree burn.Methods 60 patients with Deep Ⅱ Degree Burn diagnosed by the Department of burn and plastic surgery of the Affiliated Hospital of Chengde Medical College from March 2018 to December 2019 were randomly divided into two groups.Both groups were treated with standardized wound treatment.On this basis,the control group (n=30) was treated with iodophor solution externally,and the dressing was changed once a day.The treatment group (n=30) was treated with compound Huangbai liquid coating externally,Change dressing once a day.The levels of vascular endothelial growth factor (VEGF) and fibroblast growth factor-7 (FGF-7) in serum and wound tissue were measured by enzyme-linked immunosorbent assay.The pathological changes on day 0 and day 14 were observed,and the wound healing rate on 14 d and 21 d was calculated.Results The levels of serum VEGF and FGF-7 in the treatment group were significantly higher than those in the control group at 7 d and 14 d after injury (P<0.05).There was no significant difference between the two groups at 21 d after injury (P>0.05).At 7 d,14 d and 21 d after injury,the levels of VEGF and FGF-7 in the wound tissue of the treatment group were significantly higher than those of the control group (P<0.05).Epithelialization occurred on the 7th day after injury in the control group and the treatment group.Under light microscope,the tissue edema and inflammatory cell infiltration in the treatment group were less than those in the control group on the 14th day after injury.At 14 d and 21 d after injury,the wound healing rate in the treatment group was significantly higher than that in the control group (P<0.05).Conclusion Compound Cortex Phellodendri fluid treatment can accelerate wound healing and neovascularization,by inhibiting the production and release some growth factors,and is worth clinical promotion.
