摘　　要：目的:探讨应用自体肋软骨块进行鼻小柱基底填充的方法和手术效果。方法:2016年7月-2019年7月共收治139例鼻部形态欠佳伴鼻基底凹陷的就医者,所有就医者鼻整形的同时均使用块状肋软骨作为填充材料进行鼻基底凹陷填充。术后随访数月,评估就医者术后鼻部外形及功能恢复情况,记录并发症发生情况,调查就医者满意度。结果:经过6-36个月的随访,139例就医者的鼻基底凹陷及鼻部外形得到明显改善。无上唇麻木、影响表情等并发症发生,仅1例就医者双侧鼻翼基底填充物术后7 d诉单侧齿龈沟有异物感,行手法复位后恢复良好,效果满意。结论:在鼻整形的同时进行自体块状肋软骨填充能够改善鼻部外形及面部比例关系,并发症发生情况少,就医者术后满意度高。Objective To investigate the method and effect of filling the base of nasal small column with autogenous costal cartilage.Methods From July 2016 to July 2019,139 patients with poor nasal morphology and depression of nasal base were treated.All patients used massive costal cartilage as filling material to fill the depression of nasal base during rhinoplasty.The patients were followed up for several months to evaluate the postoperative nasal shape and functional recovery,record the occurrence of complications,and investigate the satisfaction of the patients.Results After 6 to 36 months follow-up,the depression of nasal base and nasal shape of 139 patients were significantly improved.There were no complications such as numbness of upper lip and affecting expression.Only one patient complained of foreign body sensation in unilateral gingival sulcus 7 days after bilateral nasal alar base filler.After manual reduction,it recovered well and the effect was satisfactory.Conclusion Filling with autologous massive costal cartilage while rhinoplasty can improve the shape of the nose and the proportion of the face,with less complications and high postoperative satisfaction.
