改良眼轮匝肌-提上睑肌固定法重睑成形术临床效果评价与分析

摘　　要：目的:观察改良眼轮匝肌-提上睑肌固定法重睑成形术临床应用效果。方法:选取2017年12月-2019年12月于笔者医院接受重睑成形术的103例就医者临床资料进行回顾性研究,按治疗术式不同分为传统组(n=51)、改良组(n=52)。传统组采取皮肤-提上睑肌固定法重睑成形术,改良组采取改良眼轮匝肌-提上睑肌固定法重睑成形术。比较两组围术期指标、术后并发症、美观满意度、重睑线宽度变化幅度、眼睑肿胀评分、提上睑肌肌力。结果:两组围术期出血量和术后切口Ⅰ级愈合情况对比,差异均无统计学意义(P>0.05)。改良组手术时间较传统组短,并发症发生率低于传统组,差异均有统计学意义(P<0.05)。改良组术后6个月重睑线宽度变化幅度、眼睑肿胀评分较传统组低,且术后1个月、3个月眼睑肿胀评分低于传统组,术后1、3、6个月提上睑肌肌力较传统组高(P<0.05)。改良组术后6个月美观满意度为94.23%,高于传统组的78.43%(P<0.05)。结论:临床采用改良眼轮匝肌-提上睑肌固定法重睑成形术既可缩短手术用时,又可减少就医者并发症的发生,提高上睑肌肌力,降低重睑线宽度变化幅度、眼睑肿胀程度,提升眼部美观度。Objective To observe the clinical application effect of modified orbicularis oculi muscle-levator muscle fixation double blepharoplasty.Methods A retrospective study was conducted on 103 patients who received double blepharoplasty in the author’s hospital from December 2017 to December 2019.They were divided into traditional group (n=52) and modified group (n=51) according to different treatment methods.The traditional group was treated with double eyelid plasty by skin levator palpebrae muscle fixation,and the improved group was treated with double eyelid plasty by improved orbicularis oculi muscle levator palpebrae muscle fixation.The perioperative indexes,postoperative complications,aesthetic satisfaction,change range of double eyelid line width,eyelid swelling score and muscle strength of levator palpebrae were compared between the two groups.Results There was no significant difference in the amount of perioperative bleeding and the grade I wound healing between the two groups (P>0.05).The operation time of the improved group was shorter than that of the traditional group,and the incidence of complications was lower than that of the traditional group (P<0.05).The change range of double eyelid line width and eyelid swelling score in the improved group were lower than those in the traditional group at 6 months after operation,and the eyelid swelling score at 1 and 3 months after operation was lower than that in the traditional group.The muscle strength of levator palpebrae was higher than that in the traditional group at 1,3 and 6 months after operation (P<0.05).The aesthetic satisfaction of the improved group was 94.23% at 6 months after operation,which was higher than 78.43% in the traditional group (P<0.05).Conclusion The modified orbicularis oculi-levator fixation method double blepharoplasty can shorten the operation time,reduce the complications,improve the muscle strength of the upper eyelid,reduce the width of the double eyelid line and the eyelid.The degree of swelling,enhance the beauty of the eyes.
