PNAM联合改良Millard术式对单侧完全性唇裂患儿鼻唇畸形的修复效果研究

摘　　要：目的:探究鼻牙槽塑形矫治器(PNAM)联合改良Millard术式对单侧完全性唇裂患儿鼻唇畸形的修复效果。方法:选取笔者医院2018年7月-2020年3月收治的180例单侧完全性唇裂患儿,随机分为对照组(n=90)和观察组(n=90),对照组行Millard手术治疗,观察组行Millard手术+PNAM治疗,分别比较两组手术时间、术中出血量、唇高、唇长、鼻底宽度、鼻小柱高度、牙槽骨裂隙、上唇裂隙及并发症。结果:术后观察组手术时间及术中出血量显著低于对照组,差异有统计学意义(P<0.05)。术后两组唇高及鼻底宽度均显著下降,唇长及鼻小柱高度均显著上升(P<0.05),且观察组的唇高及鼻底宽度显著低于对照组,唇长及鼻小柱高度显著高于对照组(P<0.05)。术后两组牙槽骨裂隙,上唇裂隙均明显下降,且观察组牙槽骨及上唇裂隙显著低于对照组(P<0.05);两组并发症发生率比较差异无统计学意义(P>0.05)。结论:采用PNAM联合改良Millard术式治疗单侧完全性唇裂患儿可有效缩短手术时间,改善患儿鼻唇畸形,美学修复效果显著。Objective To explore the effect of presurgical nasal molding (PNAM) combined with improved Millard operation on the repair of nasolabial deformities in children with unilateral complete cleft lip.Methods 180 childrens with unilateral complete cleft lip treated in our hospital from July 2018 to March 2020 were randomly divided into control group (n=90) and observation group (n=90).The control group was treated with Millard operation and the observation group was treated with Millard operation + PNAM.The operation time,intraoperative bleeding,lip height,lip length,nasal fundus width,columella height,alveolar bone fissure Upper lip fissure and complications.Results Postoperatively,the operative time and intraoperative bleeding in the observation group were significantly lower than those in the control group,the difference was statistically significant(P<0.05).The lip height and nasal floor width decreased significantly in both groups,and the lip length and nasal columella height increased significantly in both groups (P<0.05),and the lip height and nasal floor width in the observation group were significantly lower than those in the control group,and the lip length and nasal columella height were significantly higher than those in the control group (P<0.05).The alveolar fissure and upper lip fissure in both groups The postoperative alveolar bone fissure and upper lip fissure in both groups decreased significantly,and the alveolar bone and upper lip fissure in the observation group were significantly lower than those in the control group (P<0.05).The difference in the incidence of complications between the two groups was not statistically significant (P>0.05).Conclusion The PNAM combined with improved Millard operation in the treatment of children with unilateral complete cleft lip can effectively shorten the operation time,improve the nasolabial deformities of the children,and have a significant aesthetic repair effect.
