温经燥湿汤联合胶原贴敷料对寒湿型痤疮患者皮肤屏障功能及外周血Th1/Th2型细胞因子水平的影响

摘　　要：目的:探讨温经燥湿汤联合胶原贴敷料对寒湿型痤疮患者皮肤屏障功能及外周血Th1/Th2型细胞因子水平的影响。方法:选取2017年12月-2019年12月于笔者医院就诊的96例面部寒湿型痤疮患者作为研究对象,随机分为三组,每组32例。对照A组予以胶原贴敷料,对照B组予以温经燥湿汤,观察组予以温经燥湿汤联合胶原贴敷料。观察统计三组临床疗效、复发率及治疗前后中医证候积分、皮损评分、睾酮(T)、雌二醇(E_(2))、白介素-4(IL-4)、干扰素γ(IFN-γ)、IFN-γ/IL-4、皮肤屏障功能。结果:观察组治疗总有效率高于对照A、B组,差异有统计学意义(P<0.05);治疗2周、4周后,观察组中医证候积分、皮损评分、经皮水分丢失量低于对照A、B组,角质层含水量高于对照A、B组,差异有统计学意义(P<0.05);治疗2周、4周后,观察组T、IL-4水平低于对照A、B组,E_(2)、IFN-γ、IFN-γ/IL-4水平高于对照A、B组,差异有统计学意义(P<0.05);观察组治疗8周、12周后,复发率低于对照A、B组,差异有统计学意义(P<0.05)。结论:温经燥湿汤联合胶原贴敷料治疗寒湿型痤疮,可有效消除炎性皮疹及非炎性皮疹,调节性激素及Th1/Th2型细胞因子,促进皮肤屏障功能恢复,进一步提高治疗效果。Objective To investigate the effect of Wenjing Zaoshi decoction combined with collagen dressing on skin barrier function and peripheral blood Th1/Th2 cytokine levels in patients with cold-damp acne.Methods From December 2017 to December 2019,96 patients with facial cold-damp acne in our hospital were selected as the research objects,and they were simply randomized into 3 groups with 32 cases in each.The control group A was given a collagen patch dressing,the control group B was given Wenjing Zaoshi Decoction,and the observation group was treated with Wenjing Zaoshi Decoction combined with collagen dressing.The clinical efficacy,recurrence rate,TCM syndrome score,skin lesion score,testosterone (T),estradiol (E_(2)),interleukin-4 (IL-4),interferon-γ (IFN-γ),IFN-γ/IL-4 and skin barrier function before and after treatment were observed and counted.Results The total effective rate of treatment in the observation group was higher than that in the control groups A and B (P<0.05).After 2 weeks and 4 weeks of treatment,the TCM syndrome score,skin lesion score,and transdermal water loss of the observation group were lower than those of the control groups A and B,the water content of the stratum corneum was higher than that of the control groups A and B (P<0.05),after 2 weeks of treatment and after 4 weeks of treatment,the levels of T and IL-4 in the observation group were lower than those in the control groups A and B,and the levels of E_(2),IFN-γ and IFN-γ/IL-4 were higher than those in the control groups A and B (P<0.05),the relapse rate of the observation group was lower than that of the control groups A and B at 8 and 12 weeks after treatment (P<0.05).Conclusion The application of Wenjing Zaoshi Decoction combined with collagen dressing to treat cold-damp acne can effectively eliminate inflammatory and non-inflammatory rashes,regulate sex hormones and Th1/Th2 cytokines,promote the recovery of skin barrier function,and further improve the therapeutic effect.
