从阴阳论治难治性痤疮的经验探析

摘　　要：难治性痤疮,主要是指经连续一个月以上的常规药物治疗,痤疮皮损无改善、消退缓慢、停药后复发的痤疮。岳仁宋教授以阴阳立论,认为“寒郁”导致的阴阳失衡(“阳化气”不足与“阴成形”太过)是难治性痤疮的基本病机。针对该病机,结合“寒郁”的部位之不同,岳仁宋教授从调理阳气入手,以“伸阳”为治疗原则,郁在上焦解表透邪、郁在中焦益气健脾、郁在下焦通阳利水,并以消痞散结法贯穿治疗始终,诸法妙用,则寒郁解、伏阳伸、难治性痤疮消退,对于临床具有一定的指导意义。Refractory acne mainly refers to the acne with no improvement in skin lesions,slow regression,and recurrence after drug withdrawal after more than one month of continuous conventional drug treatment.Professor Yue Rensong uses Yin-Yang theory to diagnose and treat refractory acne,and he believes that the Yin-Yang disharmony(“Yang transforming Qi”is deficient and“Yin constituting Form”is hyperactive.)caused by“Cold-evil restraining Yang-Qi”is the basic pathogenesis of refractory acne.According to the pathogenesis of refractory acne and the body parts of Yang-Qi stagnation caused by Cold evil,Professor Yue started from regulating Yang Qi and regarded stretching Yang Qi as the therapeutic principles.His treatments include inducing sweating to releasing exterior in the upper energizer,tonifying Qi and Spleen in the middle energizer,raisingYang and resolve retained fluid in the lower energizer while a cure for acne nodules runs through the treatment.Ingenious use of various methods will make the Cold-evil restraining Yang-Qi removed,Yang-Qi extension and the refractory acne subsided which has certain guiding significance for clinical practice.
