血清CTRP5、脂联素与颈动脉粥样硬化的关系

摘　　要：目的探讨2型糖尿病(T2DM)血清C1q/肿瘤坏死因子相关蛋白5(CTRP5)、脂联素(APN)的水平与颈动脉粥样硬化(CAS)的关系。方法采用病例对照研究,纳入2019年8月至2019年11月就诊于山西医科大学第二医院的患者共125例,其中健康对照组45例,T2DM无CAS患者(T2DM组)40例及T2DM合并CAS患者(T2DM+CAS组)40例。采用酶联免疫吸附法(ELISA)检测血清CTRP5、APN水平。结果T2DM患者血清CTRP5较对照组升高(P<0.05),且T2DM+CAS组进一步升高(P<0.05)。血清APN在T2DM患者中减低(P<0.05),但在T2DM组与T2DM+CAS组间无显著差异(P>0.05);CTRP5为T2DM合并CAS的独立危险因素,可作为T2DM合并CAS的筛查指标;血清CTRP5与APN、高密度脂蛋白-胆固醇(HDL-C)、收缩压、糖化血红蛋白A1c(HbA1c)、体重指数(BMI)和空腹胰岛素(FINS)相关(P均<0.01)。结论血清CTRP5水平与糖尿病大血管病变相关,可作为T2DM是否合并CAS的预测因子。Objective To measure the level of serum C1q/tumor necrosis factor related protein 5(CTRP5)and adiponectin(APN)in type 2 diabetes mellitus(T2DM)with carotid atherosclerosis(CAS),and to investigate the relationship between CTRP5,APN and CAS.Methods In this case control study we enrolled 125 subjects who were diagnosed in the Second Hospital of Shanxi Medical University between August 2019 and November 2019,including 45 healthy control subjects(control group),40 T2DM without CAS(T2DM group),and 40 T2DM with CAS(T2DM+CAS group).Serum CTRP5 and APN were determined by ELISA.Results Serum CTRP5 level in T2DM patients was higher than that in the control group(P<0.05),and further increased in T2DM+CAS group(P<0.05).Serum APN level was significantly reduced in T2DM patients(P<0.05),but there was no significant difference between T2DM group and T2DM+CAS group(P>0.05).Serum CTRP5 level was an independent risk factor for T2DM with CAS,which might be used as a screening index for T2DM combined with CAS.Serum CTRP5 level was correlated with APN,HDL-C,SBP,HbA1c,BMI and FINS(P<0.01).Conclusion Serum CTRP5 level is associated with diabetic macroangiopathy and may serve as an independent predictor of T2DM combined with CAS.
