摘  要:目的研究传染性单核细胞增多症(infectious mononucleosis,IM)患儿血浆中微小RNA-155(micro RNA-155,mi R-155)表达水平,并分析其与EBV DNA载量的相关性。方法选取2017年3月—2021年3月我院收治的96例活动期IM患儿(活动期IM组)和81例缓解期IM患儿(缓解期IM组)作为研究对象,按照EBV DNA载量将活动期IM患儿分为EBV DNA≥3.38 lg拷贝/ml组(56例)、EBV DNA<3.38 lg拷贝/ml组(40例);选择同期在我院体检的健康儿童90例作为对照组。采用实时荧光定量PCR法检测血浆中EBV DNA载量和mi R-155表达水平,采用Pearson相关分析法分析mi R-155水平与EBV DNA载量的相关性,采用ROC曲线评估血浆中mi R-155表达水平预测IM患儿为活动期的价值。结果与对照组相比,缓解期IM组、活动期IM组血浆mi R-155水平均较高(P均<0.05);与缓解期IM组相比,活动期IM组血浆mi R-155水平较高(P<0.05)。与EBV DNA<3.38 lg拷贝/ml组相比,EBV DNA≥3.38 lg拷贝/ml组血浆mi R-155水平较高(P<0.05)。Pearson相关性分析显示,活动期IM患儿血浆mi R-155水平与EBV DNA载量呈正相关(r=0.547,P<0.05);ROC曲线显示,血浆mi R-155水平预测活动期IM患儿的AUC为0.869,其敏感度为65.90%,特异度为93.80%。结论活动期IM患儿血浆中mi R-155表达水平上调,与EBV DNA载量呈正相关,可为早期预估IM患儿病情提供依据。Objective To detect the expressional level of microRNA-155(miR-155)in the plasma of children with infectious mononucleosis(IM),and to analyze its correlation with EBV DNA load.Methods Ninety-six children with active IM and 81 children with remissive IM were recruited in Qinhuangdao first hospital from March 2017 to March 2021.Children with active IM were further divided into the EBV DNA≥3.38 lg copies/ml group(56 cases)and the EBV DNA<3.38 lg copies/ml group(40 cases)according to the EBV DNA load;meanwhile,90 healthy children who underwent physical examination in this hospital during the same period were enrolled as the control group.Real-time fluorescent quantitative PCR method was used to detect EBV DNA load and miR-155 expression level in plasma,Pearson correlation analysis was used to analyze the correlation between miR-155 expression level and EBV DNA load.The ROC curve was used to evaluate the value of plasma miR-155 level in predicting the active phase of IM children.Results Compared with the control group,the plasma miR-155 level in the remission IM group and the active IM group was higher(P>0.05),compared with the remission IM group,the plasma miR-155 level in the active IM group was higher(P<0.05);compared with the EBV DNA<3.38 lg copies/ml group,the plasma miR-155 level in the EBV DNA≥3.38 lg copies/ml group was higher(P<0.05).Pearson correlation analysis showed that there was a positive correlation between the plasma miR-155 level and EBV DNA load in children with active IM(r=0.547,P<0.05);the ROC curve showed that AUC of plasma miR-155 level predicting children with active IM was 0.869,with sensitivity of 65.90%and specificity of 93.80%.Conclusions The expression level of miR-155 in the plasma of children with active IM is up-regulated,which is positively correlated with the EBV DNA load,thus providing a basis for early diagnosis of children with IM.
