腘静脉与胫后静脉入路置管溶栓术治疗急性下肢深静脉血栓患者的效果比较

摘　　要：目的:比较腘静脉与胫后静脉入路置管溶栓术治疗急性下肢深静脉血栓患者的效果。方法:回顾性分析2016年6月至2019年10月该院收治的80例行溶栓治疗的急性下肢深静脉血栓患者的临床资料,按不同置管入路方法分为两组,45例患者经患肢腘静脉入路置管,作为腘静脉组;35例患者经患肢胫后静脉入路置管,作为胫后静脉组。比较两组血管再通率、尿激酶使用量、症状消失时间、静脉通畅程度、肢体消肿率和并发症发生率。结果:溶栓第5天,两组血管再通率、患肢静脉通畅度评分和肢体消肿率比较,差异均无统计学意义(P>0.05);两组尿激酶使用量、症状消失时间比较,差异均无统计学意义(P>0.05);腘静脉组并发症发生率明显低于胫后静脉组,差异有统计学意义(P<0.05)。结论:腘静脉与胫后静脉入路置管溶栓术治疗急性下肢深静脉血栓患者的效果相当,但腘静脉入路置管溶栓术可降低并发症发生率。Objective:To compare effects of popliteal vein and posterior tibial vein catheter thrombolysis in treatment of patients with acute deep vein thrombosis of lower limbs.Methods:The clinical data of 80 patients with acute deep vein thrombosis of lower limbs who received thrombolytic therapy from June 2016 to October 2019 were retrospectively analyzed.These patients were divided into two groups according to the different catheter placement methods.45 patients were intubated through the popliteal vein of the affected limb and were used as popliteal vein group,while 35 patients were intubated through the posterior tibial vein of the affected limb and were set as posterior tibial vein group.The revascularization rate,the urokinase dosage,the symptom disappearance time,the venous patency,the limb swelling rate and the complication rate were compared.Results:On the 5 th day of thrombolysis,there were no significant differences in the revascularization rate,the venous patency score of the affected limb,and the detumescence rate of the limbs between the two groups(P>0.05).Further,there were no significant differences in the urokinase dosage and the symptom disappearance time between the two groups(P>0.05).The incidence of complications in the popliteal vein group was significantly lower than that in the posterior tibial vein group,and the difference was statistically significant(P<0.05).Conclusions:Popliteal vein and posterior tibial vein catheter thrombolysis have similar effects in the treatment of acute deep vein thrombosis of lower limbs,but popliteal vein catheter thrombolysis can reduce the incidence of complications.
