医用弹力袜在老年下肢静脉曲张大隐静脉高位结扎剥脱术后护理中的应用效果

摘　　要：目的:观察医用弹力袜在老年下肢静脉曲张大隐静脉高位结扎剥脱术后护理中的应用效果。方法:回顾性分析2020年5-10月于该院行大隐静脉高位结扎剥脱术的45例老年下肢静脉曲张患者的临床资料,作为观察组。回顾性分析2019年9月至2020年4月于该院行大隐静脉高位结扎剥脱术的45例老年下肢静脉曲张患者的临床资料,作为对照组。对照组术后应用弹力绷带加压包扎,观察组应用医用弹力袜,比较两组视觉模拟评分法(VAS)评分、凝血功能指标[凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)]水平、下肢静脉血栓发生率、患肢小腿周径缩小值和并发症发生率。结果:术后2个月,两组VAS评分均低于术前,且观察组低于对照组,差异有统计学意义(P<0.05);术后3周,两组PT、APTT均长于术后次日,且观察组长于对照组,两组FIB水平均低于术后次日,且观察组低于对照组,差异有统计学意义(P<0.05);观察组术后下肢静脉血栓发生率为6.67%,明显低于对照组的22.22%,差异有统计学意义(P<0.05);观察组术后2、5个月患肢小腿周径缩小值均高于对照组,差异有统计学意义(P<0.05);观察组术后并发症发生率为15.56%,明显低于对照组的35.56%,差异有统计学意义(P<0.05)。结论:医用弹力袜在老年下肢静脉曲张大隐静脉高位结扎剥脱术后护理中的应用效果优于弹力绷带,可降低术后VAS评分、下肢静脉血栓发生率和并发症发生率,改善凝血功能指标水平,促进术后患肢小腿周径缩小。Objective:To observe application effects of medical elastic stockings in nursing of elderly patients with varicose veins of lower extremities undergoing high ligation and stripping of the great saphenous vein.Methods:The clinical data of 45 elderly patients with varicose veins of lower extremities undergoing high ligation and stripping of the great saphenous vein in our hospital from May to October 2020 were retrospectively analyzed,and they were selected as the observation group.The clinical data of 45 elderly patients with varicose veins of lower extremities undergoing high ligation and stripping of the great saphenous vein in our hospital from September 2019 to April 2020 were retrospectively analyzed,and they were used as the control group.After the surgery,the control group was given the elastic bandage pressure bandaging,while the observation group used the medical elastic stockings.The visual analogue scale(VAS)score,the coagulation function index levels[prothrombin time(PT),activated partial thrombin time(APTT),fibrinogen(FIB)],the incidence of lower extremity venous thrombosis,the calf circumference reduction value and the complication rate were compared between the two groups.Results:2 months after the surgery,the VAS scores of both groups were lower than those before the surgery;that of the observation group was lower than that of the control group;and the differences were statistically significant(P<0.05).3 weeks after the surgery,the PT and APTT in both groups were longer than the day after the surgery,and those of the observation group were longer than those of the control group;the levels of FIB in the two groups were lower than the day after the surgery,and that of the observation group was lower than that of the control group;and the differences were statistically significant(P<0.05).The incidence of postoperative lower extremity venous thrombosis in the observation group was 6.67%,which was significantly lower than that the control group of 22.22%,and the difference was statistically significant
