摘  要:目的探讨两种检测方法对临床常用的血红蛋白(Hb)、红细胞比容(Hct)、血钾(K^(+))、血钠(Na^(+))、血氯(Cl^(-))、血糖(Glu)检测的结果进行比较,为临床血液标本的检测提供参考。分析血气分析仪、全自动干式生化分析仪、全自动血液分析仪在不同类别样本中测定K^(+)、Na^(+)、Cl^(-)、Glu、Hb、Hct 6个相同检测项目结果的可比性。方法选择2018年3月至6月在同济大学附属杨浦医院急诊住院的157例患者,其中男性88例,女性69例;年龄41~94岁,平均年龄67.65岁。分别用丹麦雷度ABL800血气分析仪、VTROS4600全自动干式生化分析仪、XN-1000全自动血液分析仪对157例送验动脉全血和静脉血清样本进行K^(+)、Na^(+)、Cl^(-)、Glu、Hb、Hct检测。比较3种分析仪测定6个相同项目检测结果。结果ABL800血气分析仪与VTROS4600全自动干式生化分析仪、XN-1000全自动血液分析仪测定的6个相同项目分别比较,Na^(+)、Hb的检测结果差异无统计学意义(P>0.05);而Glu、K^(+)、Cl^(-)、Hct检测结果差异有统计学意义(P<0.05),其中动脉血中Glu、Cl^(-)、Hb、Hct检测结果高,K^(+)检测结果较低。回归分析表明,ABL800血气分析仪检查结果与VTROS4600全自动干式生化分析仪、XN-1000全自动血液分析仪的6个临床实验室指标检测均具有较好的相关性。通过对6个项目偏倚回归分析发现,ABL800血气分析仪检测K^(+)的偏倚与其他两种仪器检测结果均值呈负相关性(r=-0.956),而Na^(+)、Glu、Cl^(-)、Hct和Hb的偏倚则均呈正相关性(r=0.872、0.997、0.928、0.925、0.934)。结论血气分析仪测定的动脉血结果Na^(+)、Hb可以替代两种全自动的分析仪检测静脉血,但Hct、K^(+)、Cl^(-)、Glu的结果有差异,可供临床参考。Objective To compare the detection results of hemoglobin(Hb), hematocrit(Hct), blood potassium(K^(+)), blood sodium(Na^(+)), blood chlorine(Cl^(-)), and blood glucose(Glu) using two methods and provide reference for clinical blood specimen detection, and analyze comparability of blood gas analyzer, automatic dry biochemical analyzer, and automatic blood analyzer for test results of 6 identical items of K^(+), Na^(+), Cl^(-), Glu, Hb and Hct in different types of samples. Methods From March to June2018, a total of 157 patients in Emergency Department of Tongji University Affiliated Yangpu Hospital were enrolled, which included 88 males and 69 females, aged 41-94 years old with mean age of 67.65 years old. The ABL800 blood gas analyzer,VTROS4600 automatic dry biochemical analyzer and XN-1000 automatic blood analyzer were used to detect K^(+), Na^(+), Cl^(-), Glu,Hb and Hct in arterial whole blood and venous serum samples. The detection results of 6 identical items measured by 3 kinds of analyzers were compared. Results The results of 6 identical items measured by ABL800 blood gas analyzer, VTROS4600 automatic dry biochemical analyzer and XN-1000 automatic blood analyzer were compared respectively, there was no statistically significant difference in detection results of Na^(+)and Hb(P > 0.05);the differences in detection results of Glu, K^(+), Cl^(-)and Hct were statistically significant(P < 0.05), of which Glu, Cl^(-), Hb, Hct in arterial blood were high but K^(+)was low. The regression analysis showed that the results of ABL800 blood gas analyzer had good correlation with six clinical laboratory index tests of VTROS4600 automatic dry biochemical analyzer and XN-1000 automatic blood analyzer. The bias regression analysis of six items tested showed that the bias of K^(+)detected by ABL800 blood gas analyzer was negatively correlated with the mean value of that from other two instruments(r =-0.956), while bias of Na^(+), Glu, Cl^(-), Hct and Hb were positively correlated with the results from the other two instrumen
