摘 要:目的研究维生素D2注射液联合抗生素治疗社区获得性肺炎(CAP)患儿的临床疗效。方法选取2019年10月至2020年10月收治的98例CAP患儿作为研究对象,随机分为对照组和联合组,每组49例。2组均给予抗炎、退热、化痰、止咳等常规治疗。联合组在对照组基础上补充维生素D2。比较2组临床疗效、肺炎胸片吸收情况、症状体征消退时间、住院时间及治疗前后白细胞计数(WBC)、中性粒细胞比例(NEUT%)、维生素D、Ca^(2+)、肺功能[最大呼气中段流量(MMEF)、最高呼气流速(PEF)、50%潮气量时呼气流速(TEF50%)]变化。结果联合组临床疗效、肺炎胸片吸收率显著高于对照组,退热时间、咳嗽消失时间、气促消失时间、肺部啰音消失时间、住院时间显著短于对照组,差异有统计学意义(P<0.05)。2组治疗前WBC、NEUT%、维生素D、Ca^(2+)、MMEF、PEF、TEF50%比较差异无统计学意义(P>0.05);2组治疗后WBC、NEUT%均显著低于治疗前,维生素D、Ca^(2+)、MMEF、PEF、TEF50%均显著高于治疗前,差异有统计学意义(P<0.05),且联合组治疗后WBC、NEUT%低于对照组,维生素D、Ca^(2+)、MMEF、PEF、TEF50%高于对照组,差异有统计学意义(P<0.05)。结论维生素D2注射液联合抗生素治疗CAP具有良好临床疗效,能够显著减轻患儿临床症状体征,改善肺部炎性反应指标,提高患儿肺功能。Objective To investigate the therapeutic effects of vitamin D2 injection combined with antibiotics on community-acquired pneumonia(CAP)in children.Methods Ninety-eight children with CAP,who were treated from October 2019 to October 2020,were randomly divided into control group and combination treatment group,with 49 cases in each group.The patients in both groups were given conventional treatment including antiinfection,antipyretic,expectorant and relieving airway spasm.On that basis the patients in combination treatment group were treated by vitamin D2.The therapeutic effects,lung inflammation absorption rate,the time of fever,cough and rale disappearing,hospitalization time,WBC,NEUT%,vitamin D,Ca^(2+),lung function indexes(MMEF,PEF and TEF50%)before and after treatment were observed and compared between two groups.Results The clinical efficacy and lung inflammation absorption rate in combination treatment group were significantly higher than those in control group,while the antipyretic time,cough disappearance time,disappearance time of panting,disappearance time of lung rale and hospitalization time were significantly shorter than those in control group(P<0.05).Before treatment,there were no significant differences in levels of WBC,NEUT%,vitamin D,Ca^(2+),MMEF,PEF and TEF50%between the two groups(P>0.05).After treatment,the levels of WBC and NEUT%in both groups were significantly decreased,while the levels of vitamin D,Ca^(2+),MMEF,PEF and TEF50%were significantly increased(P<0.05).Moreover,decrease degree and increase degree in combination treatment group were more significant than those in control group(P<0.05).Conclusion Vitamin D2 combined with antibiotics is very effective in treating CAP,which can markedly alleviate clinical symptoms,reduce inflammatory reactions and improve the pulmonary function of patients with CAP.
