银杏达莫注射液联合甲磺酸倍他司汀片治疗眩晕的疗效

摘　　要：目的探讨银杏达莫注射液联合甲磺酸倍他司汀片(敏使朗)治疗眩晕的疗效及对脑血流量、脑氧代谢的影响。方法选择眩晕患者120例,依据随机数字表法均分成对照组和观察组,每组60例。对照组予以甲磺酸倍他司汀片(敏使朗)治疗,观察组在对照组治疗的基础上加用银杏达莫注射液,比较2组治疗后的疗效、治疗前后平均血流速度[基底动脉(BA)、右侧椎动脉(RVA)和左侧椎动脉(LVA)]、血液流变学指标[纤维蛋白原(FIB)、血浆黏度、红细胞聚集指数(AI)和全血高切黏度]、氧代谢指标[动静脉血氧差D_((a-v)O_(2))和脑氧摄取率(COER)]、眩晕障碍调查量表(DHI)评分、眩晕症状评分、血清内皮素-1(ET-1)和人降钙素基因相关肽(CGRP)水平以及治疗期间的不良反应。结果治疗后观察组总有效率高于对照组(P<0.05);观察组BA、RVA、LVA的平均血流速度、D_((a-v)O_(2))、COER和血清CGRP水平均高于对照组(P<0.05);FIB、血浆黏度、AI、全血高切黏度、血清ET-1水平、ET-1/CGRP、眩晕症状评分和DHI评分均低于对照组(P<0.05);治疗期间2组未见明显药物不良反应。结论银杏达莫注射液联合敏使朗治疗眩晕疗效较好,可明显改善眩晕症状和ET-1、CGRP水平失衡,并增大脑血流量,促进脑组织有氧代谢水平。Objective To explore the curative effect of Ginkgo-dipyridamole Injections combined with Betahistine Mesilate Tablets(Merislon)on vertigo and its influences on cerebral blood flow and cerebral oxygen metabolism.Methods A total of 120 vertigo patients were enrolled and divided into control group and observation group by random number table method,60 cases in each group.The control group were given Merislon,while the observation group were additionally given Ginkgo-dipyridamole Injections.The curative effect after treatment,mean flow velocity[basilar artery(BA),right vertebral artery(RVA),left vertebral artery(LVA)],hemorheology indexes[fibrinogen(FIB),plasma viscosity,erythrocyte aggregation index(AI),whole blood high-shear viscosity],oxygen metabolism indexes[arterio-venous oxygen content difference D_((a-v)O_(2)),cerebral oxygen extraction rate(COER)],scores of dizziness handicap inventory(DHI)and vertigo symptoms,levels of serum endothelin-1(ET-1)and human calcitonin gene-related peptide(CGRP)before and after treatment,and adverse reactions during treatment were observed and compared between the 2 groups.Results After treatment,the total response rate of observation group was higher than that of control group(P<0.05).The mean flow velocity of BA,RVA,LVA,D_((a-v)O_(2)),COER and levels of serum CGRP in observation group were higher than those in control group(P<0.05),while FIB,plasma viscosity,AI,whole blood high-shear viscosity,level of serum ET-1,ET-1/CGRP,symptoms scores and DHI score were lower than those in control group(P<0.05).There was no significant adverse drug reaction during treatment in either group.Conclusion The curative effect of Merislon combined with Ginkgo-dipyridamole Injections is good on vertigo,which can significantly improve vertigo symptoms and the imbalance of ET-1 and CGRP levels,increase the cerebral blood flow and promote the aerobic metabolism level of brain tissues.
