摘 要:目的与数字减影血管造影(digital subtraction angiography,DSA)对比,探讨自然心率下双源计算机断层扫描(computed tomography,CT)血管成像(CT angiography,CTA)在冠状动脉起源异常中的诊断价值。方法广东省人民医院珠海医院2017年8月至2020年1月在DSA检查中发现16例心脏起源异常患者,均在自然心率下条件下进行冠状动脉CTA扫描,回顾分析这16例患者的CT图像,并与DSA对比。结果16例患者的冠状动脉CT检查均能发现冠状动脉起源异常,100%与DSA结果吻合。总共行冠状动脉CTA检查的有1067例患者,剔除7例客观原因检查中断,纳入统计的共1060例,冠状动脉起源异常占冠状动脉CT检查的1.5%。CTA及DSA显示冠状动脉狭窄的结果以DSA为“金标准”,灵敏度达90%,特异度达98%,阳性预测值达90%,阴性预测值达98%。结论自然心率下双源CTA能够保证图像质量,能满足临床诊治冠状动脉起源异常需求。objectives To evaluate the diagnostic value of dual-source computed tomography(CT)angiography(CTA)in anomalous origin of coronary artery under natural heart rate compared with digital subtraction angiography(DSA).Methods From August 2017 to January 2020,16 patients with abnormal cardiac origin were detected by DSA in Guangdong Provincial People′s Hospital Zhuhai Hospital.All the patients were scanned by CTA at natural heart rate.The CT images of the 16 patients were analyzed retrospectively and compared with DSA.Results The abnormal origin of coronary artery could be found on CT images of the 16 patients,100%of which were in agreement with DSA.Of the 1067 patients who underwent coronary CTA,1060 patients were included in the total,excluding 7 patients who were interrupted due to objective reasons.The abnormal origin of coronary artery accounted for 1.5%of the total coronary CTA examination.The sensitivity,specificity,positive predictive value and negative predictive value were 90%,98%,90%and 98%,respectively.Conclusions Dual-source CTA at natural heart rate can guarantee the image quality and meet the needs of clinical diagnosis and treatment of anomalous origin of coronary artery.
