磁共振弥散加权成像联合磁共振血管成像在急性期脑梗死患者诊断中应用研究

摘　　要：目的:探讨磁共振弥散加权成像(DWI)联合磁共振血管成像(MRA)在急性期脑梗死患者诊断中应用价值。方法:选择2019年1月~2020年12月某院收治的疑似急性期脑梗死患者80例,入选者均进行DWI、MRA检查。以临床综合诊断结果作为金标准,分析DWI、MRA单项及联合检查在急性期脑梗死的应用价值,包括准确性、敏感性、特异性及阳性预测值、阴性预测值。结果:经临床综合诊断发现,80例疑似急性期脑梗死患者中确诊45例,剩余35例排除急性脑梗死的可能;DWI、MRA联合诊断急性期脑梗死的灵敏度[100.00%(45/45)]、特异度[97.14%(34/35)]、准确度[98.75%(79/80)]、阳性预测值[97.83%(45/46)]、阴性预测值[100.00%(34/34)]均高于DWI、MRA各项单独检测,差异有统计学意义(P<0.05)。结论:在急性期脑梗死诊断中使用DWI联合MRA能够提高灵敏度、特异度及准确度,临床可根据影像学表现进行准确判断,为后续治疗提供有价值的参考信息。Objective:To explore the application value of magnetic resonance diffusion weighted imaging(DWI)combined with magnetic resonance angiography(MRA)in the diagnosis of patients with acute cerebral infarction.Methods:A total of 80 patients with suspected acute cerebral infarction in a hospital from January 2019 to December 2020 were selected.All selected patients were examined by DWI and MRA.The comprehensive clinical diagnosis result was used as the gold standard to analyze the application value of DWI,MRA single item and combined examination in the acute phase of cerebral infarction,including accuracy,sensitivity,specificity,positive predictive value,and negative predictive value.Results:After comprehensive clinical diagnosis,45 cases were diagnosed in 80 patients with suspected acute cerebral infarction,and the remaining 35 cases excluded the possibility of acute cerebral infarction.The sensitivity,specificity,accuracy,positive predictive value,and the negative predictive value of DWI and MRA combined diagnosis in the acute cerebral infarction were 100.00%(45/45),97.14%(34/35),98.75%(79/80),97.83%,(45/46),and 100.00%(34/34),respectively,which were higher than those of individual detection of DWI and MRA,and the difference was statistically significant(P<0.05).Conclusion:The application of DWI combined with MRA in the diagnosis of acute cerebral infarction can improve the sensitivity,specificity and accuracy.Clinically,accurate judgments can be made based on imaging findings,which can provide valuable reference information for subsequent treatment.
