内镜下圈套器不同切除方法对结直肠息肉患者应激反应指标及炎症反应指标的影响研究

摘　　要：目的:研究内镜下圈套器不同切除方法治疗结直肠息肉的临床效果。方法:根据治疗方法的不同,将某院收治的160例结直肠息肉患者分为热切组和冷切组各80例,热切组采用内镜下圈套器热切除,冷切组采用冷切除治疗。对比两组患者的息肉完整切除率,并于手术前后分别检测血清应激反应指标(E、NE、Cor)和炎症反应指标(CRP、IL-1、IL-6),统计术后并发症的发生率。结果:热切组息肉完整切除率为93.75%,冷切组为95.00%,两组比较无统计学意义(P>0.05);术后冷切组患者血清E、NE、Cor和CRP、IL-1、IL-6均低于热切除组(P<0.05),差异存在统计学意义;冷切组共7.50%的患者出现并发症,低于热切组的18.75%(P<0.05),差异存在统计学意义。结论:针对直径不超过10mm的小息肉,内镜下圈套器热切术与冷切术所取得的息肉完整切除率相当,但冷切术治疗下可有效减轻手术创伤应激刺激和炎症反应,并降低术后并发症的发生率。Objective:To study the clinical effect of different resection methods of endoscopic snare in the treatment of colorectal polyps.Methods:According to the different treatment methods,160 patients with colorectal polyps admitted to a hospital were divided into the hot resection group(n=80)and the cold resection group(n=80),and the two groups were given endoscopic snare hot and cold resection,respectively.The complete removal rate of polyps was compared between the two groups.Serum stress response indexes(E,NE,Cor)and inflammatory response indexes(CRP,IL-1,IL-6)were detected before and after surgery,and the incidence of postoperative complications was calculated.Results:The complete polyp removal rate of the eager group was 93.75%,and that of the cold resection group was 95.00%(P>0.05).After operation,serum E,NE,Cor,CRP,IL-1 and IL-6 in cold resection group were lower than those in eager group(P<0.05).Complications occurred in 7.50%of patients in the cold resection group,lower than 18.75%in the eager group(P<0.05).Conclusion:For small polyps with diameter not more than 10mm,the complete removal rate of polyps achieved by endoscopic snare hot resection is comparable to that achieved by cold resection,but cold resection can effectively reduce surgical traumatic stress and inflammatory response,and reduce the incidence of postoperative complications.
