摘  要:目的:探讨高分辨率超声在甲状腺乳头状癌微钙化诊断中的应用效果。方法:选取2018年4月~2020年3月某院收治的甲状腺乳头状癌患者96例为研究对象,所有患者均行常规超声及高分辨超声检查,以病理检查为“金标准”,分析高分辨超声下甲状腺乳头状癌微钙化表现,评估常规超声、高分辨率超声及两者联合在甲状腺乳头状癌微钙化中的诊断效能。结果:96例患者共发现130个结节;未钙化结节41例(49个结节),微钙化55例(81个结节),微钙化结节主要包括弥漫型和局灶型;130个结节中,常规超声诊断甲状腺乳头状癌微钙化与金标准一致性K值为0.477,一致性一般;高分辨率超声诊断结果与金标准一致性K值为0.619,一致性一般;两者联合检测诊断结果与金标准一致性K值为0.749,一致性好;3种检测方法特异度对比,差异无统计学意义(P>0.05);以上3种检查方法,对比结果显示,检查敏感度、准确度最高为联合检查,其次为高分辨超声,而常规超声排名第3,差异有统计学意义(P<0.05)。结论:高分辨率超声能够提高对甲状腺乳头状癌微钙化的诊断效能,而与常规超声联合应用则能进一步提升诊断效能,值得借鉴和重视。Objective:To investigate the application of high-resolution ultrasound in the diagnosis of microcalcification in papillary thyroid carcinoma.Methods:A total of 96 cases of patients with papillary thyroid carcinoma who were screened and treated in a hospital from April 2018 to March 2020 were collected.All patients were performed conventional ultrasound and high resolution ultrasound.Using pathological examination as the"gold standard",the microcalcification performance of papillary thyroid carcinoma under high-resolution ultrasound was analyzed,and the diagnostic efficacy of conventional ultrasound,high-resolution ultrasound and the two in combination in microcalcification of papillary thyroid carcinoma was evaluated.Results:A total of 130 nodules were found in 96 patients.There were 41 cases of uncalcified nodules(49 nodules)and 55 cases of microcalcified nodules(81 nodules).Microcalcified nodules mainly included diffuse type and focal type.Among the 130 nodules,the K value of conventional ultrasound in the diagnosis of thyroid papillary carcinoma microcalcification which was consistent with the gold standard was 0.477,with general consistency.The K value of high resolution ultrasound in the diagnosis of thyroid papillary carcinoma microcalcification which was consistent with the gold standard was 0.619,with general consistency.The K value of the combined detection diagnosis which was consistent with the gold standard was 0.749,with good consistency.There was no significant difference in the specificity among the three detection methods(P>0.05).Among three inspection methods above,the comparison results showed that the highest sensitivity and accuracy of inspection was combined inspection,followed by high-resolution ultrasound,while the conventional ultrasound ranked third,and the difference was statistically significant(P<0.05).Conclusion:High resolution ultrasound can improve the diagnostic efficiency of thyroid papillary carcinoma microcalcification,and combined with conventional ultrasound can further improv
