低温等离子射频消融术与传统喉切开手术治疗早期声门型喉癌的疗效对比

摘　　要：目的探讨低温等离子射频消融术与传统喉切开手术治疗早期声门型喉癌的临床疗效。方法依据治疗方式的不同将125例早期声门型喉癌患者分为微创组(n=75)和传统组(n=50例),微创组患者实施低温等离子射频消融术治疗,传统组患者实施传统喉切开手术治疗。比较两组患者的手术相关指标、嗓音学评估参数、嗓音功能、并发症发生情况及术后生存情况。结果微创组患者的手术时间、住院时间均明显短于传统组,术中出血量明显少于传统组,术后24 h视觉模拟评分法(VAS)评分明显低于传统组,差异均有统计学意义(P﹤0.01)。术后1年,两组患者的基频、基频微扰、振幅微扰均高于本组术前,微创组患者的基频、基频微扰、振幅微扰均低于传统组,差异均有统计学意义(P﹤0.05)。术后1年,两组患者嗓音障碍指数量表(VHI)中功能、生理、情感评分均低于本组术前,且微创组患者的功能、生理、情感评分均低于传统组,差异均有统计学意义(P﹤0.05)。微创组患者的并发症总发生率为8.00%,明显低于传统组患者的28.00%(P﹤0.01)。两组患者的1、2、3年生存率比较,差异均无统计学意义(P﹥0.05)。结论与传统喉切开手术相比,低温等离子射频消融术治疗早期声门型喉癌具有手术时间短、术中出血量少、创伤性小等优点,利于患者术后嗓音功能恢复,且并发症较少。Objective To explore the clinical efficacy of hypothermic plasma radiofrequency ablation and traditional laryngotomy in the treatment of early glottic laryngeal cancer.Method 125 patients with early glottic laryngeal cancer were divided into minimally invasive group(n=75,treated with hypothermic plasma radiofrequency ablation)and traditional group(n=50,treated with traditional laryngotomy).The operation related indexes,phonology parameters,voice function,complications and postoperative survival were compared between the two groups.Result The operation time and hospital stay were shorter in the minimally invasive group than those in the traditional group,the intraoperative blood loss and scores of visual analogue scale(VAS)at 24 hours after operation in the minimally invasive group were lower than those in the traditional group,the differences were statistically significant(P<0.01).One year after operation,the fundamental frequency,fundamental frequency perturbation,amplitude perturbation in the two groups were higher than those before operation,and the above indicators in the minimally invasive group were lower than those in the traditional group,the differences were statistically significant(P<0.05).One year after operation,the scores of function,physiology and emotion in voice handicap index(VHI)in the two groups were lower than those before operation,with lower scores in the minimally invasive group than those in the traditional group(P<0.05).The incidence of complications in the minimally invasive group was significantly lower than that in the traditional group(8.00%vs 28.00%,P<0.01).There was no significant difference in 1-,2-and 3-year survival rates between the two groups(P>0.05).Conclusion Compared with traditional laryngotomy,hypothermic plasma radiofrequency ablation has the advantages of shorter operation time,less intraoperative blood loss and less trauma in the treatment of early glottic laryngeal cancer,which is beneficial to the recovery of postoperative voice function and lower complications.
