加味麻桂五苓五皮汤治疗风寒闭肺型急性肾炎临床观察

摘　　要：目的:观察加味麻桂五苓五皮汤治疗风寒闭肺型急性肾炎的临床疗效。方法:选取急性肾炎患者90例,采用随机数字表法分为两组各45例。对照组予常规抗菌、利尿、降压等治疗,并嘱患者注意休息与合理饮食;治疗组在对照组基础上予加味麻桂五苓五皮汤加减,两组均治疗4周,观察两组患者的临床疗效及24 h尿蛋白定量、白细胞介素-2 (IL-2)、白细胞介素-6(IL-6)在治疗前后的变化。结果:治疗组总有效率为93.2%,对照组总有效率为83.7%,差异具有统计学意义(P﹤0.05)。治疗后,治疗组24 h尿蛋白定量为(0.83±0.32) g,对照组为(1.33±0.25) g,两组比较,差异具有统计学意义(P﹤0.05);两组IL-2水平较治疗前明显升高,IL-6水平较治疗前明显降低,差异有统计学意义(P﹤0.05),组间比较,治疗组改善更明显,差异有统计学意义(P﹤0.05)。结论:加味麻桂五苓五皮汤联合西药治疗风寒闭肺型急性肾炎临床疗效较好,可以明显改善患者的临床症状,以及24 h尿蛋白定量、IL-6、IL-2等指标。Objective:To observe the clinical efficacy of modified Magui Wuling Wupi decoction on acute nephritis with syndrome of wind-cold blocking lung.Methods:90 cases of selected patients with acute nephritis were divided into two groups by random number table,45 cases in each group.The control group was treated by routine anti-bacteria,diuresis and hypotension etc.,and the patients were instructed to pay attention to rest and reasonable diet;while the treatment group was treated plus with modified Magui Wuling Wupi decoction on the basis of control group.Both groups were treated for 4 weeks.The clinical efficacy and the changes of 24-hour urine total protein quantity(24 hUTP),interleukin-2(IL-2)and interleukin-6(IL-6)of patients were observed in two groups before and after treatment.Results:The total effective rate was 93.2% in the treatment group and 83.7% in the control group,and the difference was statistically significant(P ﹤ 0.05).After treatment,24 hUTP of treatment group was(0.83±0.32)g,while24 hUTP of control group was(1.33±0.25)g,and the comparative difference between two groups was statistically significant(P﹤0.05);the levels of IL-2 level in both groups were significantly higher than those before treatment,while IL-6 levels were significantly lower than those before treatment,and the differences were statistically significant(P﹤0.05).The treatment group was more significantly improved than control group(P ﹤ 0.05).Conclusion:The clinical effect of modified Magui Wuling Wupi decoction and western medicine on acute nephritis with syndrome of wind-cold blocking lung is significant,which can not only significantly improve the clinical symptoms of patients,but also significantly reduce the levels of 24 hUTP and IL-6 of patients,and increase the level of IL-2.
