益肾养血通窍汤对慢性耳鸣患者耳鸣严重程度及睡眠质量的影响

摘　　要：目的探讨益肾养血通窍汤对慢性耳鸣患者的耳鸣严重程度及睡眠质量的影响。方法选取2018年1月-2019年12月新疆医科大学附属中医医院收治的慢性耳鸣患者88例,用随机数字表法将病例分成两组,每组各44例。对照组应用常规西药治疗,治疗组在常规西药治疗基础上联合益肾养血通窍汤治疗。比较治疗后两组患者临床疗效、耳鸣严重程度、中医证候疗效、睡眠质量评分,并测定两组患者γ氨基丁酸(GABA)、五羟色胺(5-HT)的水平。结果治疗后,治疗组临床总有效率为90.9%(40/44),明显高于对照组的81.8%(36/44),差异有统计学意义(P<0.05);可明显减轻患者耳鸣的严重程度,改善睡眠质量;治疗组中医证候疗效也明显优于对照组(P<0.05)。治疗后,治疗组GABA水平明显高于对照组(P<0.05),而5-HT水平明显低于对照组(P<0.05)。结论在西医常规治疗的基础上联合益肾养血通窍汤治疗可显著提高慢性耳鸣患者的临床疗效,减轻耳鸣的严重程度和改善睡眠质量,其作用机制可能与肾养血通窍汤能提高GABA水平及降低5-HT水平有关。Objective To investigate the effect of Yishen Yangxue Tongqiao Decoction on the severity of tinnitus and sleep quality in patients with chronic tinnitus. Methods 88 patients with chronic tinnitus admitted to our hospital from January 2018 to December 2019 were selected and randomly divided into two groups(with different treatment methods),44 patients in each group. The control group was treated with conventional western medicine,and the research group was treated with Yishen Yangxue Tongqiao Decoction on the basis of control group. The effective rate,tinnitus severity,TCM syndromes,γ-aminobutyric acid(GABA),5-hydroxytryptamine(5-HT)and sleep quality were compared between two groups. Results After treatment,the effective rate of the research group(90.9%,40/44)was higher than that of control group(81.8%,36/44)(P<0.05). In the research group,the treatment improved quality of sleep,and the patients’ overall perception of tinnitus scores were lower than those in the control group(P<0.05);the severity of tinnitus in the treatment group was lower than that of the control group(P<0.05). The scores of TCM syndromes in the research group were lower than those in the control group(P<0.05). GABA level in the research group was higher than that in the control group(P<0.05),and 5-HT in the research group was lower than that in the control group(P<0.05). The sleep quality score of the research group was lower than that of the control group(P<0.05). Conclusion In the treatment of chronic tinnitus, adding Yishen Yangxue Tongqiao Decoction to conventional western medicine treatment can significantly improve the clinical efficacy,alleviate the severity of tinnitus,and improve quality of sleep. The mechanism could be related to the increase of GABA level and decrease of 5-HY levle by Yishen Yangxue Tongqiao Decoction.
