原发性痛风患者疾病不确定感与整体疼痛的现状调查及相关性分析

摘　　要：目的调查了解原发性痛风患者疾病不确定感与整体疼痛评估现状,及二者之间的相关性。方法2020年8月-2021年1月便利抽取山东省青岛大学附属医院痛风临床医学中心就诊的原发性痛风患者270例,采用基本情况调查表、疾病不确定感成人量表(MUIS-A)、整体疼痛评估量表(GPS)进行问卷调查,并进行疾病不确定感现状、整体疼痛现状的相关性分析。结果共回收有效问卷257份,有效回收率95.19%。原发性痛风患者的疾病不确定感量表总得分为(93.03±14.74)分,处于中等水平;整体疼痛评估量表总得分为(80.32±39.65)分,疼痛情况欠佳,原发性痛风患者的疾病不确定感与疼痛呈正相关(r=0.392,P<0.01)。回归分析结果显示:文化程度(β=0.225,P<0.001)、病程(β=0.176,P=0.002)、疾病分期(β=0.153,P=0.006)和整体疼痛(β=0.365,P<0.001)是原发性痛风患者疾病不确定感的重要影响因素。结论原发性痛风患者疾病不确定感水平不容乐观,疼痛是其疾病不确定感的主要影响因素,疼痛情况越严重,疾病不确定感水平越高。医护人员应密切关注原发性痛风患者的疼痛情况,并以科学的方法缓解疼痛,以减轻其疾病不确定感。Objective To investigate the current situation of illness uncertainty and global pain assessment in patients with primary gout and to explore their correlation.Methods From August 2020 to January 2021,270 primary gout patients were selected as objects from a gout clinical medical center in the Affiliated Hospital of Qindao University by convenient sampling method.The survey was carried out by means of the general questionnaire,Mishel Uncertainty in Illness Scale for Adult(MUIS-A),and Global Pain Scale(GPS).The correlation analysis of illness uncertainty status and global pain assessment status was conducted.Results A total of 257 valid questionnaires were collected,and the effective recovery rate was 95.19%.In this study,the total score of MUIS-A of primary gout patients was(93.03±14.74),which was in the middle level,and the total score of GPS was(80.32±39.65),which the pain assessment was poor.There was a positive correlation between illness uncertainty and pain in patients with primary gout(r=0.392,P<0.01).The results of multiple stepwise regression analysis showed that education(β=0.225,P<0.001),course of disease(β=0.176,P=0.002),disease stage(β=0.153,P=0.006),and pain(β=0.365,P<0.001)were important influencing factors of illness uncertainty in patients with primary gout.Conclusion The level of illness uncertainty is not optimistic,and pain is the main influencing factor of illness uncertainty in patients with primary gout.The more severe the pain,the higher the level of illness uncertainty.Nurses shall give more attention to the pain of patients with primary gout and adopt scientific methods to relieve it,so as to reduce their sense of illness uncertainty.
