中药熏蒸联合造口引流术及术后抗感染治疗前庭大腺囊肿疗效观察及对患者负性情绪、生活质量的影响

摘　　要：目的:观察中药熏蒸联合造口引流术及术后抗感染治疗前庭大腺囊肿的疗效及对患者负性情绪、生活质量的影响。方法:选择82例前庭大腺囊肿患者,随机分为观察组与对照组各41例,对照组接受造口引流手术及术后抗感染治疗,观察组在对照组的基础上接受中药熏蒸治疗。比较2组治疗前后中医证候积分及焦虑自评量表(SAS)、抑郁自评量表(SDS)、健康状况调查简表(SF-36)评分,并比较2组临床疗效。结果:治疗后观察组总有效率95.12%,高于对照组80.49%(P<0.05)。与同组治疗前比较,治疗后2组疼痛、肿胀、会阴不适以及阴道分泌物增多等中医证候积分及SAS、SDS评分均降低(P<0.05),生理健康总评分(PCS)、心理健康总评分(MCS)升高(P<0.05);与对照组治疗后比较,观察组治疗后疼痛、肿胀、会阴不适以及阴道分泌物增多等中医证候积分及SAS、SDS评分均较低(P<0.05),PCS、MCS评分较高(P<0.05)。结论:中药熏蒸联合造口引流术及术后抗感染治疗前庭大腺囊肿可以有效缓解患者的临床症状,提高临床疗效,同时改善患者的焦虑、抑郁情绪,提高生活质量。Objective:To observe the curative effect of Chinese medicine fumigatin combined with ostomy for drainage and postoperative anti-infective therapy on Bartholin’s cyst and its effect on negative emotions and quality of life of patients.Methods:A total of 82 cases of patients with Bartholin’s cyst were selected and randomly divided into the observation group and the control group,with 41 cases in each group.The control group was given ostomy for drainage and postoperative antiinfective therapy,and the observation group was additionally treated with Chinese medicine fumigation based on the treatment of the control group.Before and after treatment,Chinese medicine syndrome scores,and scores of Self-rating Anxiety Scale(SAS),Self-rating Depression Scale(SDS)and Short Form 36 Health Survey(SF-36)were compared between the two groups.Clinical effects in the two groups were compared.Results:After treatment,the total effective rate was95.12%in the observation group,higher than that of 80.49%in the control group(P<0.05).When compared with those in the same group before treatment,scores of SAS,SDS and Chinese medicine syndromes including pain,swelling,perineal discomfort and increased vaginal discharge after treatment in the two groups were decreased(P<0.05),and scores of Physical Component Summary(PCS)and Mental Health Component Summary(MCS)were increased(P<0.05).After treatment,scores of SAS,SDS and Chinese medicine syndromes including pain,swelling,perineal discomfort and increased vaginal discharge in the observation group were lower than those in the control group(P<0.05),and scores of PCS and MCS were higher(P<0.05).Conclusion:Chinese medicine fumigatin combined with ostomy for drainage and postoperative anti-infective therapy for Bartholin’s cyst can effectively relieve clinical symptoms,anxiety and depression of patients,and improve the clinical effect and quality of life.
