氢吗啡酮硬膜外自控镇痛泵治疗难治性癌痛的疗效观察

摘　　要：目的探讨氢吗啡酮硬膜外自控镇痛泵治疗难治性癌痛患者的有效性和安全性。方法选取2018年3月-2020年12月在广西壮族自治区江滨医院住院治疗的30例难治性癌痛患者,使用硬膜外自控镇痛泵入氢吗啡酮治疗,比较患者治疗前1天及治疗后第1、3、7、30天疼痛评分、药物不良反应以及生活质量。结果治疗后第1、3、7、30天,患者疼痛评分均较治疗前1天降低,差异有统计学意义(P<0.05);治疗后第1、3天,患者恶心呕吐、便秘及头晕的发生率降低,但差异无统计学意义(P>0.05),治疗后第7、30天,患者恶心呕吐、便秘及头晕的发生率低于治疗前1天,差异有统计学意义(P<0.05);治疗7 d后,患者生活质量评分高于治疗前,差异有统计学意义(P<0.05)。结论对于难治性癌痛患者,硬膜外自控镇痛泵入氢吗啡酮能减轻疼痛、改善患者生活质量,是一种有效、安全、简便易行的方法。Objective To observe the clinical efficacy and safety of hydromorphone by self-controlled epidural analgesia pump in treament of refractory cancer pain.Methods A total of 3030 patients with refractory cancer pain who were hospitalized in Jiangbin Hospital of Guangxi from March 2018 to December 2020 were selected and treated with hydromorphone by self-controlled epidural analgesia pump.The pain score,adverse drug reactions and quality of life of the patients were compared one day before treatment and 1,3,7 and 30 days after treatment.Results On the 1st,3rd,7th and 30th day after treatment,the pain scores were lower than those on the 1st day before treatment,and the difference was statistically significant(P<0.05).On the 1st and 3rd day after treatment,the incidence of nausea,vomiting,constipation and dizziness decreased,but the difference was not statistically significant(P>0.05).On the 7th and 30th day after treatment,the incidence of nausea,vomiting,constipation and dizziness was lower than that on the 1st day before treatment,and the difference was statistically significant(P<0.05).After 7 days of treatment,the quality of life score was higher than that before treatment,and the difference was statistically significant(P<0.05).Conclusion Hydromorphone by self-controlled epidural analgesia pump can significantly reduce the cancer pain in patients with refractory cancer pain,improve patients'quality of life.It is an effective,safe and simple method.
