摘  要:目的:通过Meta分析系统评价壳多糖酶3样蛋白1(CHI3L1)基因rs4950928多态性与哮喘发病风险的相关性。方法:计算机检索知网、万方、维普、CBM、PubMed、Web of Science、EMbase和The Cochrane Library数据库,查找国内外关于CHI3L1基因rs4950928多态性与哮喘相关性的病例-对照研究,检索时限均为从建库至2021年1月。由2位研究者独立按纳入与排除标准筛选文献、提取资料并评价纳入研究的方法学质量后,采用Stata15.0软件进行Meta分析和发表偏倚评估。结果:共纳入13篇病例-对照研究,Meta分析结果显示:CHI3L1基因rs4950928多态性与哮喘易感性有关[GG vs CG+CC:OR=1.291,95%CI(1.045,1.595),P=0.018;GG vs CC:OR=1.300,95%CI(1.036,1.631),P=0.023]。按年龄进行亚组分析结果显示,CHI3L1基因rs4950928多态性与成人哮喘易感性有关[GG vs CG+CC:OR=1.443,95%CI(1.107,1.880),P=0.007;GG vs CC:OR=1.430,95%CI(1.074,1.903),P=0.014],但在儿童组中未发现该多态性与哮喘存在统计学关联。按种族进行亚组分析结果显示,CHI3L1基因rs4950928多态性与亚洲人群哮喘易感性有关[GG vs CG+CC:OR=1.475,95%CI(1.072,2.029),P=0.017;G vs C:OR=1.246,95%CI(1.042,1.491),P=0.016;GG vs CC:OR=1.811,95%CI(1.235,2.655),P=0.002],但在高加索人群中未发现该多态性与哮喘存在统计学关联。结论:CHI3L1基因rs4950928多态性与哮喘易感性相关,GG基因型相对CC或CG基因型,可能会增加哮喘发病风险,特别是在成人及亚洲人群中。在亚洲人群中,G等位基因可能是哮喘的易感因子,C等位基因可能是哮喘的保护因子,在高加索或儿童人群中,CHI3L1基因rs4950928多态性可能与哮喘易感性不相关。Objective:To explore the genetic association between rs4950928 of CHI3 L1 and asthma susceptibility by Meta analysis.Methods:Databases including CNKI,Wanfang,VIP,CBM,PubMed,Web of Science,EMbase and The Cochrane Library were systemically searched to collect between CHI3 L1 rs4950928 polymorphism and asthma case-control studies published to January2021.Two independent reviewers screened literatures according to the inclusion and exclusion criteria,extracted data,and evaluated methodological quality of included studies.Meta analyses as well as publication bias were performed using Stata 15.0.Results:A total of 13 case-control studies were included in our analysis.Meta association was found between CHI3 L1 rs4950928 polymorphism and asthma susceptibility[GG vs CG+CC:OR=1.291,95%CI(1.045,1.595),P=0.018;GG vs CC:OR=1.300,95%CI(1.036,1.631),P=0.023].When stratified by age,there was significant association between CHI3 L1 gene rs4950928 polymorphism and asthma susceptibility in adult population[GG vs CG+CC:OR=1.443,95%CI(1.107,1.880),P=0.007;GG vs CC:OR=1.430,95%CI(1.074,1.903),P=0.014].However,no significant association was found among CHI3 L1 rs4950928 polymorphism and asthma susceptibility in children population.When stratified by ethnicity,there was significant association between rs4950928 of CHI3 L1 and asthma susceptibility in Asian population[GG vs CG+CC:OR=1.475,95%CI(1.072,2.029),P=0.017;G vs C:OR=1.246,95%CI(1.042,1.491),P=0.016;GG vs CC:OR=1.811,95%CI(1.235,2.655),P=0.002].However,the association among CHI3 L1 rs4950928 polymorphism and asthma susceptibility was not significant in Caucasian population.Conclusion:The CHI3 L1 gene rs4950928 polymorphism is a susceptibility locus of asthma.GG genotype might significantly increase the risk of asthma,compared with CC or CG genotype,especially in adult and Asian population.In Asian population,G allele may be susceptible factor for asthma,and C allele may be a protective factor for asthma.There may be no association between CHI3 L1 gene rs4950928 polymorphism and a
