摘　　要：目的:探讨核苷酸结合寡聚化结构域样受体6(NLRP6)在未足月胎膜早破(PPROM)患者合并组织学绒毛膜羊膜炎(HCA)诊断中的临床价值。方法:选取2020年12月至2021年6月于徐州医科大学附属徐州妇幼保健院住院分娩的PPROM患者60例,其中未合并HCA的PPROM组33例、合并HCA的PPROM组27例;另选取50例足月正常分娩孕妇未合并HCA作为对照组。收集入院时孕妇的血清样本、分娩时新鲜胎膜组织标本、入院时血液中C反应蛋白(CRP)及白细胞计数的表达情况。ELISA法检测血清NLRP6、caspase-1水平,免疫组化法及Western blot法检测胎膜组织中NLRP6蛋白表达,实时荧光定量PCR(RT-qPCR)技术检测胎膜组织中NLRP6及caspase-1 mRNA表达水平。结果:3组孕妇血液中CRP及白细胞计数比较,差异均无统计学意义(P>0.05)。免疫组化法检测显示,NLRP6主要表达于胎膜上皮细胞、间充质细胞的细胞质,部分表达于细胞核中。Western blot法及RT-qPCR法检测结果均显示,3组孕妇胎膜中NLRP6及caspase-1蛋白及mRNA相对表达量比较,差异有统计学意义(P<0.05);PPROM组表达量均高于对照组,其中合并HCA的PPROM组与对照组和未合并HCA的PPROM组比较差异有统计学意义(P<0.05)。ELISA法检测显示,3组孕妇血清中NLRP6及caspase-1表达水平比较,差异有统计学意义(P<0.05);PPROM组表达量均高于对照组,其中合并HCA的PPROM组和未合并HCA的PPROM组比较差异有统计学意义(P<0.001),PPROM组孕妇血清NLRP6及caspase-1水平呈正相关(r=0.8395,P<0.001)。结论:NLRP6、caspase-1在孕妇体内表达水平升高可能与HCA的发生发展有关。Objective:To investigate the value of NLRP6 in the diagnosis of preterm premature rupture of membranes(PPROM)with histological chorioamnionitis(HCA).Methods:A total of 60 patients with PPROM who delivered in Xuzhou Maternal and Child Health Hospital Affiliated to Xuzhou Medical University from December 2020 to June 2021 were selected,including 33 patients in the PPROM group without HCA and 27 patients in the PPROM group with HCA,50 pregnant women with normal term delivery without HCA were selected as control group.Serum samples of pregnant women at admission and fresh fetal membrane samples at delivery were collected,and serum NLRP6 was detected by ELISA,and expression of CRP and white blood cell count in blood at admission were collected.The expression of NLRP6 protein in membranes was detected by immunohistochemical and Western blot.The expressions of NLRP6 and caspase-1 mRNA in membranes were detected by real-time quantitative.Results:There was no significant difference in the expression of CRP and white blood cell count in blood(P>0.05).The immunohistochemical showed that NLRP6 and caspase-1 were mainly expressed in the cytoplasm of fetal membrane epithelial cells and mesenchymal cells,partly expressed in the cell nucleus.The results of Western blot and RT-PCR showed that the relative expression levels of NLRP6 and caspase-1 protein and mRNA in fetal membrane of pregnant women in the three groups were statistically different(P<0.05).The expression level of PPROM group was higher than that of control group,and the difference in the ratio of control group,PPROM without HCA group and PPROM with HCA group has statistical significance(P<0.05).ELISA showed that the levels of NLRP6 and caspase-1 in serum of pregnant women in the 3 groups were statistically significant(P<0.05).The expression level of PPROM group was higher than that of control group.There was statistical significance between PPROM without HCA group and PPROM with HCA(P<0.001).There was a positive correlation between NLRP6 and caspase-1 levels in PPRO
