佛山地区全血细胞计数结果的一致性和可比性调查

摘　　要：目的分析佛山地区全血细胞计数结果的一致性和可比性,为本区域临床诊疗提供更可靠的临床检验结果。方法基于互联网+模式调查了2019年佛山地区223家不同级别医疗机构临床实验室进行全血细胞计数结果。将2个批号调查品各参数重复检测5次(剔除离群值),对白细胞(WBC)计数、红细胞(RBC)计数、血红蛋白(Hgb)、血小板(PLT)计数进行统计分析。结果2个批号调查品WBC、RBC、Hgb、PLT结果变异系数分别为3.24%和3.43%、1.42%和1.51%、1.45%和1.34%、3.84%和3.67%。2个批号调查品各参数检测结果依据《全国临床检验操作规程》和美国临床实验室标准化协会(CLSI)H26-A2标准的符合率分别为>75%与>90%。将仪器品牌分为五组(A、B、C、D、E),五组各参数检测结果均值与所有品牌组均值差异分别为:WBC计数检测结果为-2.18%~0.36%与-2.76%~0.00%,RBC计数检测结果为-3.56%~0.11%与0.04%~3.11%,Hgb检测结果为-1.53%~0.13%与-1.15%~0.13%,PLT计数检测结果为-5.52%~0.14%与-0.25%~3.18%,提示在2个批号调查品五组各参数检测结果均存在一定的差异(P<0.05)。结论佛山地区区域性全血细胞计数结果室间质评的临床检验的质量总体趋于一致性和可比性,但不同仪器组全血细胞计数结果存在一定的差异。Objective To analyze the consistency and comparability of the complete blood count results in Foshan,and provide more reliable clinical test results in clinical diagnosis and treatment in this region.Methods Based on the Internet+model,we investigated the results of complete blood counts conducted by 223 clinical laboratories of different levels of medical institutions in Foshan in 2019.The parameters of the two batches of survey products were repeatedly tested 5 times(eliminating outliers),and the results including the items of white blood cell count(WBC),red blood cell count(RBC),hemoglobin(Hgb),and platelet count(PLT)were analyzed statistically.Results The coefficients of variation of WBC count,RBC count,Hgb,and PLT count were 3.24%and 3.43%,1.42%and 1.51%,1.45%and 1.34%,3.84%and 3.67%.The compliance rates of the test results of the two batches of survey products of each parameter in the"National Clinical Laboratory Procedures"and the American Clinical Laboratory Standards Institute(CLSI)H26-A2 standard were>75%and>90%.The instrument brands are divided into 5 groups(A,B,C,D,E).The difference in the average of the detection results of each parameter among the 5 groups and the total average of all instrument groups were as follows:WBC count,-2.18%to 0.36%and-2.76%to 0;RBC count,-3.56%to 0.11%and 0.04%to 3.11%;Hgb,-1.53%to 0.13%and-1.15%to 0.13%;PLT count,-5.52%to 0.14%and-0.25%to 3.18%.There were certain differences in the test results of each parameter in the 5 groups of the 2 batches investigated(P<0.05).Conclusion The quality of the clinical tests of the regional quality assessment of the regional complete blood count results in Foshan is generally consistent and comparable,but there are certain differences in the complete blood count results of different instrument groups.
