四妙勇安汤联合贝前列素钠片治疗湿热毒盛型下肢动脉硬化闭塞症临床研究

摘　　要：目的观察四妙勇安汤联合贝前列素钠片治疗湿热毒盛型下肢动脉硬化闭塞症(ASO)的临床疗效。方法将98例湿热毒盛型ASO患者按照随机数字表法分为2组,对照组49例予贝前列素钠片治疗,治疗组49例在对照组的基础上联合四妙勇安汤治疗。2组治疗2个月后统计疗效,比较2组治疗前后血流动力学指标(包括血管内径、血流量、峰值流速、搏动指数、阻力指数)、血脂指标[包括甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]水平变化,比较2组治疗期间不良反应发生情况。结果治疗组总有效率91.84%(45/49),不良反应发生率8.16%(4/49),对照组分别为75.51%(37/49)、22.45%(11/49),治疗组总有效率高于对照组(P<0.05),不良反应发生率低于对照组(P<0.05)。与本组治疗前比较,2组治疗后血管内径、血流量、峰值流速均增加(P<0.05),搏动指数、阻力指数均减小(P<0.05),且治疗组治疗后血流动力学各指标改善均优于对照组(P<0.05)。与本组治疗前比较,2组治疗后TG、TC、LDL-C水平均降低(P<0.05),HDL-C水平均升高(P<0.05),且治疗组治疗后血脂各项指标改善均优于对照组(P<0.05)。结论四妙勇安汤联合贝前列素钠片治疗湿热毒盛型ASO疗效确切,可明显改善患者血流动力学指标,降低血脂水平,减少不良反应,提高临床疗效。Objective To observe the clinical efficacy of Simiao Yongan Decoction with beraprost sodium tablets for lower extremity arteriosclerosis obliterans(ASO)of damp heat toxin type.Methods Totally 98 patients with lower extremity ASO(damp heat toxin type)randomly assigned into equal group,in basis of beraprost sodium tablets,treatment group additionally received Simiao Yongan Decoction.The treatment period was 4 weeks,aiming to compare hemodynamics indexes(vascular internal diameter,blood flow,peak velocity,pulsatility index,resistance index),blood lipid indexes[triglyceride(TG),total cholesterol(TC),high density lipoprotein-cholesterol(HDL-C),low density lipoprotein-cholesterol(LDL-C)];the curative effect and adverse reaction were counted.Results The total effective rates were better in treatment group than in control group[91.84%(45/49)vs 75.51%(37/49),(P<0.05),respectively].The incidence of adverse reactions 8.16%(4/49)in treatment group,was lower than that in control group 22.45%(11/49)(P<0.05).After treatment,vascular internal diameter,blood flow,peak velocity in groups were increased(P<0.05),while pulsatility index and resistance index were decreased(P<0.05),improvement of hemodynamics indexes in treatment group were better in comparison with control group(P<0.05);TG,TC,LDL-C in groups were decreased(P<0.05),HDL-C was increased(P<0.05),improvement of blood lipid indexes in treatment group were significantly better(P<0.05).Conclusion For patients with lower extremity ASO(damp heat toxin type),Simiao Yongan Decoction with beraprost sodium tablets has a definite curative effect,is significantly capable of improving the hemodynamics indexes and clinical efficacy,reducing blood lipid levels and adverse reactions.
