针刺联合肿意膏外敷治疗耳带状疱疹致周围性面瘫临床疗效观察

摘　　要：目的观察针刺联合肿意膏外敷治疗耳带状疱疹致周围性面瘫的临床疗效。方法将82例耳带状疱疹致周围性面瘫患者按照随机数字表法分为2组。对照组41例予常规西药治疗,治疗组41例采用针刺联合肿意膏外敷治疗。2组疗程均20 d。比较2组治疗前后简化McGill疼痛问卷(SF-MPQ)[疼痛分级指数(PRI)、疼痛视觉模拟评分(VAS)、现有疼痛强度(PPI)]评分和血清免疫球蛋白E(IgE)、基质金属蛋白酶9(MMP-9)、P物质(SP)水平、Portmann评分、面部残疾指数(FDI)[躯体功能(FDIP)评分、社会生活功能(FDIS)评分],并统计2组疗效和不良反应情况。结果治疗组疗效优于对照组(P<0.05)。治疗后2组PRI评分、疼痛VAS及PPI评分均较本组治疗前降低(P<0.05),且治疗后治疗组均低于对照组(P<0.05)。治疗后2组血清IgE水平较本组治疗前升高(P<0.05),MMP-9、SP水平均降低(P<0.05),且治疗后治疗组血清IgE水平高于对照组(P<0.05),MMP-9、SP水平均低于对照组(P<0.05)。治疗后2组Portmann评分、FDIP评分均较本组治疗前升高(P<0.05),FDIS评分均降低(P<0.05),且治疗后治疗组Portmann评分、FDIP评分均高于对照组(P<0.05),FDIS评分低于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论针刺联合肿意膏外敷治疗耳带状疱疹致周围性面瘫疗效确切,能改善面瘫及疼痛症状,提高患者生活质量,值得推广。Objective To observe the clinical effect of acupuncture combined with external application of Zhongyi Paste for herpes zoster oticus(HZO)with peripheral facial paralysis(PFP).Methods Totally 82 HZO patients with PFP were randomly divided into equal group,in addition to conventional Western medicine,and treatment group received acupuncture combined with external application of Zhongyi Paste.The treatment period was 20 days,aiming to compare Short-Form McGill Pain Questionnaire(SF-MPQ)[pain rating index(PRI),Visual Analogue Scale(VAS)for pain,present pain intensity(PPI)],serum immunoglobulin E(IgE),matrix metallopeptidase 9(MMP-9),Substance P(SP),Portmann scale,Facial Disability Index(FDI)[Facial Disability Index-Physical(FDIP),Facial Disability Index-Social(FDIS)];the curative effect was counted.Results The curative effect was better in treatment group than in control group(P<0.05).After treatment,SF-MPQ scores(PRI score,VAS and PPI score)in groups were decreased(P<0.05),which decreased in treatment group when compared with control group(P<0.05);serum IgE in groups was increased(P<0.05),treatment group was higher than control group(P<0.05);MMP-9 and SP in groups were decreased(P<0.05),treatment group was decreased in comparison with control group(P<0.05);Portmann scale and FDIP score in groups were increased(P<0.05),which were higher in treatment group than in control group(P<0.05);FDIS was decreased(P<0.05),treatment group was lower than control group(P<0.05).The difference was not statistically significant in the incidence of adverse reactions between groups(P>0.05).Conclusion For HZO patients with PFP,acupuncture combined with external application of Zhongyi Paste has a definite curative effect,which is capable of improving the symptoms of facial paralysis and pain,and quality-of-life,worthy of promotion.
