多巴胺+硝普钠治疗心梗后左心衰患者的有效性分析

摘　　要：目的 讨论心肌梗死(心梗)后左心衰竭(左心衰)患者运用多巴胺+硝普钠治疗方案的临床有效性。方法 100例心梗后左心衰患者,按照抽选顺序分为单一组及联合组,每组50例。单一组应用硝普钠治疗,联合组选择多巴胺联合硝普钠治疗。比较两组心功能指标[左室收缩末期内径(LVESD)、左室舒张末期内径LVEDD、左室射血分数(LVEF)]、6 min步行试验距离、治疗效果、不良反应发生情况及满意度。结果 联合组LVESD(24.51±4.36)mm、LVEDD(52.08±4.23)mm短于单一组的(37.68±7.92)、(59.82±7.69)mm, LVEF(59.12±8.57)%高于单一组的(46.48±5.19)%, 6 min步行试验距离(470.61±10.82)m长于单一组的(440.94±9.08)m,差异有统计学意义(P<0.05)。联合组治疗总有效率98.00%高于单一组的86.00%,不良反应发生率4.00%低于单一组的22.00%,差异有统计学意义(P<0.05)。联合组总满意度100.00%高于单一组的84.00%,差异有统计学意义(P<0.05)。结论 多巴胺联合硝普钠治疗心梗后左心衰患者效果显著,能够改善心功能指标,提高运动耐量,且安全性高,值得临床推广应用。Objective To discuss the clinical effectiveness of dopamine and sodium nitroprusside in the treatment of patients with left heart failure after myocardial infarction.Methods A total of 100 patients with left heart failure after myocardial infarction were divided into single group and combined group according to the selection order,with 50 cases in each group.The single group was treated with sodium nitroprusside,and the combined group was treated with dopamine in combination with sodium nitroprusside.Both groups were compared in terms of cardiac function indexes[left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF)],6-min walk test distance,therapeutic effect,occurrence of adverse reactions and satisfaction.Results The LVESD(24.51±4.36)mm and LVEDD(52.08±4.23)mm of the combined group were shorter than(37.68±7.92)and(59.82±7.69)mm of the single group,and the LVEF(59.12±8.57)%was higher than(46.48±5.19)%of the single group,and the 6-min walk test distance(470.61±10.82)m was longer than(440.94±9.08)m of the single group.All the differences were statistically significant(P<0.05).The total effective rate of treatment 98.00%in the combined group was higher than 86.00%of the single group,and the incidence of adverse reactions 4.00%was lower than 22.00%of the single group.The difference was statistically significant(P<0.05).The satisfaction rate 100.00%in the combined group was higher than 84.00%in the single group,and the difference was statistically significant(P<0.05).Conclusion Sodium nitroprusside combined with dopamine has a significant effect on patients with left heart failure after myocardial infarction,which can improve the cardiac function index and exercise tolerance with high satery,which is worth of clinical promotion and application.
