全身麻醉和椎管内麻醉在骨科手术患者中的应用效果观察

摘　　要：目的 研究全身麻醉和椎管内麻醉在骨科手术患者中的应用效果。方法 100例骨科手术患者,随机分为对照组和观察组,各50例。对照组采取全身麻醉,观察组采取椎管内麻醉。对比两组麻醉药物用量、术后睁眼时间、意识清醒时间及术后情况。结果 观察组麻醉药物用量(1630.61±101.20)ml少于对照组的(1908.03±144.06)ml,术后睁眼时间(2.51±0.51)min、意识清醒时间(4.11±0.51)min均短于对照组的(3.71±0.71)、(8.93±0.63)min,差异具有统计学意义(P<0.05)。观察组认知功能评分(26.21±2.06)分高于对照组的(19.33±1.53)分,认知障碍发生率8.00%低于对照组的24.00%,不良反应发生率22.00%低于对照组的42.00%,差异均具有统计学意义(P<0.05)。结论 椎管内麻醉相比于全身麻醉在临床应用中效果更佳,可以缩短患者的术后苏醒时间与意识清醒时间,帮助患者尽早恢复精神状态,降低不良反应的发生率,具有较高的安全性,值得推广使用。Objective To study the application effect of general anesthesia and intraspinal anesthesia in patients undergoing orthopedic surgery.Methods A total of 100 patients undergoing orthopedic surgery were randomly divided into control group and observation group,with 50 cases in each group.The control group received general anesthesia,and the observation group received intraspinal anesthesia.The dosage of narcotic drugs,postoperative eye opening time,conscious time and postoperative situation were compared between the two groups.Results The dosage of narcotic drugs(1630.61±101.20)ml of the observation group was less than(1908.03±144.06)ml of the control group,and the postoperative eye opening time(2.51±0.51)min and the conscious time(4.11±0.51)min were shorter than(3.71±0.71)and(8.93±0.63)min of the control group.All the differences were statistically significant(P<0.05).The cognitive function score(26.21±2.06)points of the observation group was higher than(19.33±1.53)points of the control group;the incidence of cognitive impairment 8.00%of the observation group was lower than 24.00%of the control group;the incidence of adverse reactions 22.00%of the observation group was lower than 42.00%of the control group;all the differences were statistically significant(P<0.05).Conclusion Compared with general anesthesia,intraspinal anesthesia has better effect in clinical application,which can shorten the postoperative recovery time and conscious time of patients,help patients recover their mental state as soon as possible,reduce the incidence of adverse reactions.It has high safety,and is worthy of promotion and application.
