摘  要:目的比较FiberWire缝线Nice结技术和微型钢板治疗掌骨干长斜形骨折的临床疗效。方法对2017年8月-2019年12月收治的65例掌骨干长斜形骨折临床资料进行回顾性对照研究。65例分别采用FiberWire缝线Nice结技术(31例)和微型钢板(34例)进行手术治疗。比较两组手术时间、骨折愈合时间、手指关节总体活动度(TAM)及Quick-DASH评分。结果两组术后均获得12~48周随访,两组随访时间差异无统计学意义(P>0.05);缝线组的手术时间及骨折愈合时间显著短于微型钢板组(P<0.05);两组在手指关节总体活动度(TAM)、Quick-DASH评分上比较差异无统计学意义(P>0.05)。结论FiberWire缝线Nice结技术治疗掌骨干长斜形骨折,其固定强度能达到骨折内固定的要求,可早期功能训练,手术时间短,骨折愈合快,无需二次手术取出。Objective To observe the effect of Fiber-Wire suture-Nice knot technology and miniplate for the treatment of long oblique metacarpal fracture.Methods We performed a retrospective analysis involving 65 patients with long oblique metacarpal fracture from August 2017 to December 2019.The patients were assigned to FiberWire suture fixation group(31 cases)and miniplate fixation group(34 cases).We evaluated the clinical effect by comparing operating time,fracture healing time,TAM score and Quick-Dash score of two groups.Results There were no significant differences in the follow-up time between two groups of all patients at the latest follow-up(range 12-48 weeks)(P>0.05).There were significant diffiences in spending time of surgery and healing time of fractures between two groups(P<0.05).It was much shorter in spending time of surgery and healing time of fractures in group FiberWire suture fixation than that in guoup of miniplate fixation.At the final follow-up,total active motion and Quick-DASH score were higher in group of miniplate fixation than that in group of FiberWire suture fixation with no significant diffiences(P>0.05).Conclusion The FiberWire suture-Nice knot technology is a reliable internal fixation for treatment of long oblique metacarpal fracture with early functional exersice.The method can shorten the time of operation and fracture healing,and keep the internal fixator from taking away by secondary surgery.
