双束纽扣带袢钢板治疗肩锁关节脱位的疗效分析

摘　　要：目的分析双束纽扣带袢钢板(Tightrope系统)固定肩锁关节及原位重建喙锁韧带治疗肩锁关节脱位的疗效。方法2017年2月-2020年2月采用双束纽扣带袢钢板做内固定材料治疗16例急性肩锁关节脱位的患者。结果16例均获得随访,随访时间10~24个月,平均12.5个月。术后肩锁关节复位良好,无关节疼痛,无喙突、锁骨骨折,无内固定断裂;内固定牢固、稳定、有效,无关节复位丢失。采用肩关节Karlsson进行评价:优12例,良4例,优良率100%。结论采用双束纽扣带袢钢板治疗肩锁关节脱位,行内固定的同时重建喙锁韧带,形成弹性固定,恢复了肩锁关节的生物环境及力学结构,且手术时间短,创伤小,临床操作不复杂,是一种有效的内固定方法,疗效可靠。Objective To analyze the efficacy of internal fixation of the acromioclavicular joint and in situ reconstructive treatment of acromioclavicular joint dislocation with Tightrope plate.Methods From February 2017 to February 2020,a total of 16 patients with acute acromioclavicular joint dislocation were treated with Endobutton plate as an internal fixation material.Results All 16 patients were followed up for 10-24 months,and the shoulder joint was evaluated by Karlsson and Constant-Murley:Excellent in 12 cases,good in 4 cases;the excellent and good rate was 100%.There was no joint pain,no rostral or clavicular fracture,and no fracture of the internal fixation;the internal fixation was firm,stable,and effective,and there was no loss of joint repositioning.Conclusion The treatment of acromioclavicular joint dislocation with Endobutton plate was carried out with internal fixation and reconstruction of the rostral ligament to form an elastic fixation,which restored the biological environment and mechanical structure of the acromioclavicular joint.It is an effective method of internal fixation with reliable efficacy and short operation time,little trauma and uncomplicated clinical operation.
