外伤性与退变性肩袖损伤治疗效果的对比分析

摘　　要：目的比较外伤性和退变性肩袖损伤镜下修复的临床疗效。方法分析2016年1月-2019年1月因外伤和退变导致的220例肩袖损伤临床资料。记录两组的平均手术时间、平均出血量,采用VAS、UCLA肩关节功能评分进行评价。结果平均手术时间、平均出血量外伤组均少于退变组,差异具有统计学意义(P<0.05)。所有患者均获13~18个月随访,随访期间未发生内固定物松动及肩袖再撕裂等并发症。外伤组UCLA评分术前平均得分(18.50±1.5)分,术后3个月平均得分(34.5±1.5)分;退变组UCLA评分术前平均得分(12.3±8.5)分,术后3个月平均得分(31.5±6.5)分。外伤组VAS评分由术前8.5~6.5分改善至术后3周1.5~0.5分及末次随访时0.8~0.5分。退变组VAS评分由术前9.5~8.5分改善至术后3周4.5~0.5分及末次随访时0.8~0.5分。结论外伤性肩袖损伤术后恢复期短,预后较退变性好,在临床诊疗中具有一定的指导意义。Objective To compare the clinical effect of microscopical repair of traumatic and degenerative rotator cuff tears.Methods The clinical data of 220 cases of rotator cuff tears caused by trauma and degeneration from January 2016 to January 2019 were analyzed.The average operation time and blood loss of two groups were recorded and evaluated by VAS and UCLA shoulder function score.Results The average operation time and blood loss in the trauma group were less than those in the degeneration group,and the difference was statistically significant(P<0.05).All patients were followed up for 13 to 18 months.No complications such as loosening of internal fixation and rotator cuff retears occurred during the follow-up period.The average preoperative UCLA score of trauma group was(18.5±1.5),the average score of 3 months after operation was(34.5±1.5)points.The average preoperative UCLA score of the degeneration group was(12.3±8.5),and the average score was(31.5±6.5).In the trauma group,the VAS score was improved from(8.5~6.5)preoperatively to(1.5~0.5)3 weeks postoperatively and(0.8~0.5)at the last follow-up.In the degenerative group,the VAS score was improved from(9.5~8.5)preoperatively to(4.5~0.5)3 weeks postoperatively and(0.8~0.5)at the last follow-up.Conclusion The recovery period of traumatic rotator cuff tears is short,and the prognosis is better than degeneration,which has a certain guiding significance in clinical diagnosis and treatment.
