拇长伸肌腱自发性断裂19例的临床治疗

摘　　要：目的探讨拇长伸肌腱自发性断裂的病因及治疗方法。方法2015年5月-2020年1月,采用示指固有伸肌腱转位及掌长肌腱游离移植对19例拇长伸肌腱自发性断裂进行修复。结果术后19例获随访4~24个月,平均6个月。拇指指间关节背伸0°~5°,屈曲35°~45°。按SEEM评分标准,示指固有伸肌腱移位重建拇长伸肌腱12例,优7例,良4例,可1例,优良率92%;掌长肌腱游离移植修复拇长伸肌腱7例,优3例,良3例,可1例,优良率86%。结论采用示指固有伸肌腱转位及掌长肌腱游离移植修复拇长伸肌腱自发性断裂,疗效确切。Objective To approach the etiology and treatment of spontaneous rupture of the extensor pollicis longus tendon.Methods Nineteen patients of spontaneous rupture of the extensor pollicis longus tendon were treated from May 2015 to January 2020 with the extensor indicis proprius tendon transfer and palmaris longus tendon grafting.Results Followed up for 4 months to 2 years of 19 cases,average 6months.The extension of interphalangeal joint of the thumb was 0°~5°,flextion was 35°~45°.Based on the SEEM,the method of the extensor indicis proprius tendon transfer was excellent in 7 cases,good in 4 cases.The method of palmaris longus tendon grafting was excellent in 3 cases,good in 3 cases.Conclusion The extensor indicis proprius tendon transfer and palmaris longus tendon grafting are valid method for treatment of spontaneous rupture of the extensor pollicis longus tendon.
