摘　　要：目的探讨顺行旋股外侧动脉降支穿支皮瓣修复腹股沟皮肤软组织缺损的疗效。方法2014年6月-2020年1月收治腹股沟区皮肤软组织缺损患者16例,创面面积:6 cm×13 cm~11 cm×25 cm,9例为单纯皮肤软组织缺损,6例合并有主干血管、神经损伤,1例合并有主干血管、神经损伤及骨折。皮瓣切取面积:6 cm×13 cm~12 cm×25 cm,以旋股外侧动脉降支为血管蒂的股前外侧皮瓣修复创面,骨折采取内固定或外固定,主干血管、神经损伤采用显微外科技术修复。供区直接闭合或植皮。结果术后皮瓣顺利成活,伤口均一期愈合,16例均获得随访,随访时间6~24个月(平均12个月);皮瓣质地柔软,弹性可,色泽与周围皮肤相似,外形满意。术后1年根据1954年英国医学研究会的感觉评价标准评定,皮瓣感觉恢复至S2~S3。结论以旋股外侧动脉降支为蒂的顺行股前外侧皮瓣是修复腹股沟区皮肤软组织缺损的有效方法之一。Objective To discuss the effectiveness of antegrade anterolateral thigh perforator flap on treatment of skin and soft tissue defects in the groin area.Methods From June 2014 to January 2020,16 patients with skin and soft tissue defects in the groin area were treated.The area of defects ranged from 6 cm×13 cm to 11 cm×25 cm,and 9 cases were mere skin and soft tissue defects,6 cases combined with nerves and vessels injury,1 cases combined with nerves,vessels injury and bone fractures.The area of perforator flaps ranged from 6 cm×13 cm to 12 cm×25 cm,and the anterolateral thigh propeller flaps were used to repair the wound.Fractures were fixed internally or externally,and the main vessel and nerve were repaired by microsurgical technique.The defects on the donor sites were closed directly or by medium thick skin graft.Results All the flaps were survival without infection and vascular crisis,the wounds of recipient and donor sites healed at first intention.16 patients were followed up from 6 to 24 months(mean,12 months),and the flaps were characterized by soft texture,good color,and satisfactory appearance.According to the Britain'S Medical Research Council at 12 months after operation,the sensation of the flaps were recovered to S2~S3.Conclusion The antegrade anterolateral thigh flap pedicled with artery descending branch of lateral circumflex femoral perforator is one of the effective methods for repairing skin and soft tissue defects in the groin area.
