中心静脉胸腔内置管持续引流治疗结核性胸腔积液的临床效果

摘　　要：目的探讨中心静脉胸腔内置管持续引流治疗结核性胸腔积液的临床效果。方法选取泰安市肿瘤防治院自2017年3月至2019年3月收治的结核性胸腔积液患者66例,根据治疗方案分为对照组和观察组,对照组24例在全身抗结核治疗基础上单纯常规胸腔抽液,观察组42例在全身结核治疗基础上给予中心静脉胸腔内置管持续引流治疗。比较两组患者治疗临床效果及不良反应发生情况。结果观察组患者治疗后的总有效率为97.62%(41/42),明显高于对照组的79.17%(19/24),差异有统计学意义(P<0.05)。观察组的不良反应总发生率为7.14%(3/42),显著低于对照组的58.33%(14/24),差异有统计学意义(P<0.001)。结论中心静脉胸腔内置管持续引流治疗结核性胸腔积液创伤小,能彻底引流积液,临床效果显著,不良反应发生率低,值得应用和推广。Objective To investigate the clinical effect of continuous drainage of central venous intrathoracic catheter in the treatment of tuberculous pleural effusion.Methods A total of 66 cases of tuberculous pleural effusion were admitted to Tai′an City Tumor Prevention Hospital from March 2017 to March 2019 were selected,and they were divided into the control group and the observation group according to the treatment plan.The control group(n=24)was treated with single conventional pleural drainage on the basis of systemic anti-tuberculosis treatment,while the observation group(n=42)was treated with continuous drainage of central venous intrathoracic catheter on the basis of systemic anti-tuberculosis treatment.The clinical effects and adverse reactions of the two groups were observed and compared.Results The total effective rate in the observation group was 97.62%(41/42),which was significantly higher than that of 79.17%(19/24)in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the observation group was 7.14%(3/42),which was significantly lower than that of 58.33%(14/24)in the control group,and the difference between groups was statistically significant(P<0.001).Conclusion In the treatment of tuberculous pleural effusion,continuous drainage of central venous intrathoracic catheter is less invasive,can completely drain the effusion,and has a significant clinical effect and low incidence of adverse reactions.Therefore,it is worthy of application and promotion.
