摘　　要：目的:结合病理表现,探讨无性细胞瘤的超声特征,以提高对该病的诊断水平。方法:回顾性分析我院2013年1月至2021年6月经手术及病理证实的10例卵巢无性细胞瘤患者的超声资料,观察肿瘤的部位、大小、形态、边缘、回声、血流信号。结果:10例中,右侧6例,左侧3例,双侧1例。7例为单纯型,3例为混合型。超声未见明显异常包块者1例,肿块表现为实性软组织肿块5例,实性部分为主的囊实性肿块4例。瘤体大多边界清楚。3例合并子宫直肠窝积液,2例合并腹腔积液。6例血流丰富,1例血流稀少(合并蒂扭转),2例未见明显血流信号(1例合并蒂扭转,1例混合型畸胎瘤成分为主)。结论:单纯型卵巢无性细胞瘤超声下多表现为巨大实性肿块,呈不均质等或低回声,内可伴散在不规则液性区/钙化,形态多较规则,边界清。彩色多普勒血流成像:肿块内血流多丰富,合并蒂扭转时血流信号稀少或未见明显血流信号。由于混合型卵巢无性细胞瘤的超声表现缺乏特异性,此时结合年龄、糖类抗原(CA)⁃125及人绒毛膜促性腺激素(HCG)水平升高对其定性诊断有帮助。Purpose:To discusse the ultrasonographic features of asexual cell tumor in combination with pathological manifestations,in order to improve the diagnostic level of asexual cell tumorwere d.Methods:The ultrasonic data of 10 patients with ovarian asexual cell tumor confirmed by surgery and pathology in our hospital from January 2013 to April 2021 were retrospectively analyzed,and the location,size,shape,margin,echo and blood flow signal of the tumor were observed.Results:In 10 cases,all cases were single.There were 6 cases on the right side and 3 cases on the left side,1 case on the both side.7 cases were simple type and 3 cases were mixed type.The mass presented as solid soft tissue mass in 5 cases and solid part mainly cystic solid mass in 4 case.Most tumors are well bounded.There were 3 cases with uterorectal fossa effusion and 2 cases with peritoneal effusion.6 patients had abundant blood flow,1 patient had sparse blood flow(with pedicle torsion),and 2 patients had no obvious blood flow signal(1 patient with pedicle torsion,and 1 patient with mixed teratoma component).Conclusion:The ultrasound imaging of simple ovarian asexual cell tumor is usually presented as a large solid mass,which is heterogeneous or hypoechoic,and can be scattered in regular liquid areas,and its morphology is irregular and boundary is clear.Color Doppler blood flow imaging shows:the blood flow in the mass is abundant,and the blood flow signal is rare or not obvious in patients with the pedicle torsion.The ultrasound finding of mixed ovarian asexual cell tumor lacks specificity.Combined with age,elevated carbohydrate antigen(CA)-125 and human chorionic gonadotropin(HCG)are helpful for qualitative diagnosis.
