摘 要:目的分析急性早幼粒细胞白血病(APL)患儿CD33、CD13及髓过氧化物酶(MPO)表达水平及临床意义。方法选择2019年1月至2020年7月陕西省汉中市三二○一医院收治的130例APL患儿为研究对象,另选取同期收治的非APL的急性髓系白血病(AML)患儿130例为对照,两组均进行骨髓穿刺,予以细胞形态学、遗传学及免疫学检查,并采用流式细胞仪行CD33、CD13和MPO检测,分析两组CD33、CD13和MPO表达水平,并探讨不同疗效的APL患儿CD33、CD13和MPO表达水平。结果以CD33及CD13≥20%、MPO≥10%为阳性表达,APL组CD33、CD13、MPO表达等级均高于对照组,差异有统计学意义(χ^(2)值分别为21.221、5.330、28.850,P<0.05);APL组CD33、CD13、MPO表达阳性率均高于对照组,差异有统计学意义(χ^(2)值分别为6.356、4.127、9.931,P<0.05);以阳性率≥80%为强阳性,完全缓解(CR)组患儿CD33、MPO表达强阳性率与非CR组比较差异不显著(P>0.05),CD13表达强阳性率高于非CR组(χ^(2)=3.949,P<0.05)。结论APL患儿CD33、CD13、MPO呈高表达,CD33、CD13、MPO表达水平检测对临床诊断APL和疗效评估具有重要意义。Objective To analyze the expression levels and clinical significance of CD33,CD13 and myeloperoxidase(MPO)in children with acute promyelocytic leukemia(APL).Methods From January 2019 to July 2020,130 children with APL,who were admitted to the 3201 Hospital of Hanzhong City,were selected as the research subjects,and 130 children with non-APL acute myeloid leukemia(AML),who were admitted during the same period,were selected as the control group.Both groups were subjected to bone marrow aspiration,cell morphology,genetics and immunology were examined,and CD33,CD13 and MPO were detected by flow cytometry.The expression levels of CD33,CD13 and MPO in the two groups were analyzed and the expression levels of CD33,CD13 and MPO in different efficacy groups were discussed.The expression levels of CD33,CD13,and MPO in children with curative APL.Results The positive rates of CD33 and CD13≥20%and MPO≥10%were deemed as positive expression.The expression levels of CD33,CD13 and MPO in the APL group were higher than those in the control group,and the differences were statistically significant(χ^(2)=21.221,5.330 and 28.850,respectively,P<0.05).The positive expression rates of CD33,CD13,and MPO in the APL group were higher than those in the control group,and the differences were statistically significant(χ^(2)=6.356,4.127 and 9.931,respectively,P<0.05).The positive rate≥80%was deemed as strong positive.Compared the strong positive expression rates of CD33 and MPO of children in the complete remission(CR)group and those in the non-CR group,the differences were not significantly different(P>0.05),and the strong positive expression rate of CD13 was higher than that of the non-CR group(χ^(2)=3.949,P<0.05).Conclusion CD33,CD13 and MPO are highly expressed in children with APL.The detection of CD33,CD13 and MPO expression levels is of great significance for clinical diagnosis of APL and evaluation of efficacy.
