代谢减重手术个案管理护士岗位设置及效果评价

摘　　要：目的:设置代谢减重手术个案管理护士岗位,并评价其效果。方法:基于我院代谢减重手术病人就诊流程和管理需求,制定代谢减重手术个案管理护士聘用条件、岗位职责、工作范围、工作流程、随访管理等相关内容,统计2019年11月—2020年9月干预实施期间登记完成率、追踪完整率、留治率、失联率、转院率、中断率,统计术前、术后1个月、术后6个月、术后1年病人体重及体质指数,调查病人满意度及个案管理护士职业满意情况。结果:我院干预实施期间设立专职个案管理护士1名,共管理53例病人,登记完成率为100.00%,追踪完整率为96.22%,留治率为92.45%,失联率为3.77%,转院率为0,中断率为7.55%;病人在术前、术后1个月、术后6个月、术后1年的体重和体质指数呈逐渐下降趋势,差异有统计学意义(P<0.001);病人对个案管理护士的工作满意度评价中,9个条目非常满意和满意占比之和均为100.00%;个案管理护士职业满意度高。结论:个案管理护士在代谢减重手术病人管理中发挥较大作用,有利于保证病人长期体重管理质量,提高护理工作质量,受到病人及护士认可。Objective:To set up the case management nurse position of metabolic bariatric surgery,and evaluate its effect.Methods:Based on treatment process and management needs of patients with metabolic bariatric surgery in our hospital,employment conditions,job responsibilities,work scope,work process,follow-up management of case management nurse of metabolic bariatric surgery were formulated.Registration completion rate,tracking complete rate,retention rate,loss of contact rate,transfer rate and interruption rate during the period of intervention implementation from November 2019 to September 2020 were statistied.The weight and body mass index of patients before surgery,1 month after surgery,6 months after surgery and 1 year after surgery were statistied.Satisfaction of patients and case management nurses were investigated.Results:1 full-time case management nurse was set up during the period of intervention implementation,and a total of 53 patients were managed.Registration completion rate was 100.00%,tracking complete rate was 96.22%,retention rate was 92.45%,loss of contact rate was 3.77%,transfer rate was 0 and interruption rate was 7.55%.Weight and body mass index of patients before surgery,1 month after surgery,6 months after surgery and 1 year after surgery were gradually decreased,and the difference was statistically significant(P<0.001).The sum of rates of very satisfied and satisfied of 9 entries in evaluation of patients'satisfaction for job of case management nurses all was 100.00%.Case management nurse had high professional satisfaction.Conclusions:Case management nurse plays important role in patient management of metabolic bariatric surgery,which is conducive to ensuring quality of patient long-term weight management,improve quality of nursing work,and been approved by patients and nurses.
