摘  要:目的开发应用静脉血栓栓塞症(Venous Thrombo Embolism,VTE)智能评估与辅助决策系统,提升院内VTE防治能力。方法基于自然语言处理对院内多元系统数据进行后结构化处理,通过机器学习方法,综合已有量表指标和专家意见,研制VTE风险评估模型;结合规则引擎、人工智能引擎,开发VTE智能评估和防治决策支持系统。结果应用该系统后,院内VTE风险发生评估率由53.8%上升到98.8%,出血风险评估率由9.8%上升到56.7%,评估时间由3.2 min/患者缩减到0.5 min/患者,恰当预防率由12.2%提高到63.4%,每筛查1例阳性患者需要检查人数由11.6人下降为3.0人。结论VTE智能评估与辅助决策系统可提升患者VTE风险预警能力,规范临床诊疗行为,对降低医院VTE发生风险、保障患者安全起到重要促进作用。Objective To develop and apply venous thrombo embolism(VTE)intelligent assessment and decision-making support system to improve the prevention and treatment capabilities to VTE in hospital.Methods Based on natural language processing,the data of multivariate system in the hospital were processed in a post-structured way.The VTE risk assessment model was developed by machine learning method,integrating the existing scale indicators and expert opinions.Combining rule engines and artificial intelligence engines,we developed VTE intelligent assessment and decision-making support system.Results After the application of this system,the assessment rate of VTE risk in hospital increased from 53.8%to 98.8%.The assessment rate of bleeding risk from 9.8%to 56.7%.The assessment time was reduced from 3.2 min/patient to 0.5 min/patient The appropriate prevention rate increased from 12.2%to 63.4%.The number of people required to be screened for each positive patient decreased from 11.6 to 3.0.Conclusion The system can improve risk early warning capability to VTE and standardize clinical behaviors.It plays an important role in decreasing VTE risk and ensuring patient’s safety.
