高功率消融与常规功率消融导管射频消融治疗心房颤动患者疗效及复发情况比较

摘　　要：目的探讨高功率消融与常规功率消融导管射频消融治疗心房颤动的临床疗效及术后复发情况。方法选取2019年8月至2020年12月本院收治的117例心房颤动患者,根据手术方式不同,60例行高功率消融导管射频消融治疗患者为A组,57例行常规功率消融导管射频消融治疗患者为B组,比较两组心脏电生理指标、手术时间、辐射剂量指标、并发症以及复发情况。结果A组手术时间、计量面积乘积、累计皮肤表面入射剂量、透视时间以及峰值皮肤剂量水平均低于B组,差异有统计学意义(P<0.05);A组并发症发生率8.33%,B组并发症发生率12.28%,差异无统计学意义(P>0.05);随访三个月A组复发率6.67%,B组复发率8.77%,差异无统计学意义(P>0.05)。结论高功率消融与常规功率消融导管射频消融治疗心房颤动其临床疗效相当,但高功率消融导管射频消融手术时间更短,可降低辐射损害。Objective To investigate the clinical effects and postoperative recurrence of high power ablation and routine power ablation catheter radiofrequency ablation in the treatment of patients with atrial fibrillation.Methods A total of 117 patients with atrial fibrillation who were treated in our hospitalwere enrolled in this study between August 2019 and December 2020.Among the patients,60 were treated with high power catheter radiofrequency ablation(group A),and other 57 were treated with routine power catheter radiofrequency ablation(group B).The cardiac electrophysiology indexes,operation time,radiation dose indexes,complications,and recurrence were compared between the two groups.Results The operation time in the group A was shorter than that in the group B(P<0.05).The product of measurement area,incident dose tinvolved skins surface,fluoroscopy time and peak skin dose level were lower in the group A than those in the group B(P<0.05).The difference in incidence of complications between the two groups was not statistically significant(8.33%vs.12.28%,P>0.05).The difference in recurrence rate after 3 months of follow-up between the two groups was also not statistically significant(6.67%vs.8.77%,P>0.05).Conclusion High power ablation and conventional power ablation catheter radiofrequency ablation have a similar clinical effect in the treatment of atrial fibrillation.However,the high power catheter radiofrequency ablation has shorter operation time,thus,it may reduce radiation damage.
