MIPPO双钢板固定治疗Ⅵ型胫骨平台骨折患者的效果分析

摘　　要：目的:探讨MIPPO双钢板固定治疗Ⅵ型胫骨平台骨折患者的效果。方法:选取2019年3月-2020年4月佳木斯市中医院收治的Ⅵ型胫骨平台骨折患者76例,按照随机数字表法分为两组,对照组应用单钢板加空心螺钉治疗,研究组应用MIPPO双钢板固定治疗。比较两组胫骨平台后倾角(posterior slope angle,PSA)、内翻角(TPA)、骨折愈合时间、术中出血量、手术时间、健康调查简表(the MOS item short from health survey,SF-36)评分、纽约特种外科医院(hospital for special surgery,HSS)膝关节评分、屈膝最大角度、伸膝最大角度。结果:研究组术后12个月PSA、TPA均低于对照组(P<0.05);研究组骨折愈合时间短于对照组(P<0.05),两组术中出血量及手术时间比较,差异均无统计学意义(P>0.05);术后6个月,研究组SF-36、HSS评分均高于对照组(P<0.05);术后6个月,研究组屈膝最大角度高于对照组,伸膝最大角度低于对照组(P<0.05)。结论:MIPPO双钢板固定治疗Ⅵ型胫骨平台骨折,可有效降低愈合时间,临床上应当进一步推广应用。Objective:To investigate the effect of MIPPO double plate fixation in the treatment of type Ⅵ tibial plateau fracture.Method:A total of 76 patients with type Ⅵ tibial plateau fracture in Jiamusi Hospital of Traditional Chinese Medicine from March 2019 to April 2020 were selected and divided into two groups according to the random number table method.The control group was treated with single plate and cannulated screw,and the study group was treated with MIPPO double plate fixation.Posterior slope angle (PSA),tibial plateau varus angle (TPA),fracture healing time,intraoperative bleeding,operation time,the MOS item short from health survey (SF-36)score,hospital for special surgery (HSS) knee score,Maximum knee flexion angle and maximum knee extension angle were compared between the two groups.Result:The levels of PSA and TPA in the study group were lower than those in the control group 12 months after operation (P<0.05);the fracture healing time of the study group was shorter than that of the control group (P<0.05),and there were no significant differences in intraoperative blood loss and operation time (P>0.05);6 months after operation,SF-36 and HSS scores of the study group were higher than those of the control group (P<0.05);6 months after operation,the maximum knee flexion angle of the study group was higher than that of the control group,and the maximum knee extension angle of the study group was lower than that of the control group (P<0.05).Conclusion:MIPPO double plate fixation in the treatment of type Ⅵ tibial plateau fractures can effectively reduce the healing time,which should be further promoted in clinical application.
