摘　　要：目的:探讨直行式锐针扣眼穿刺法在透析患者内瘘中的应用效果。方法:选取2020年5月-2021年5月在广东省农垦中心医院门诊接受维持性血液透析治疗的152例患者为研究对象。按随机数字表法将其分为观察组与对照组,各76例。观察组采用直行式锐针扣眼穿刺法进行内瘘穿刺,对照组采用传统锐针穿刺法进行内瘘穿刺。比较两组穿刺相关指标、疼痛程度、不良反应发生情况和对内瘘肢体美观度满意情况。结果:观察组一次穿刺成功率高于对照组(P<0.05);两组穿刺后血流量、静脉压和透析治疗效率(KT/V)比较,差异均无统计学意义(P>0.05)。观察组无疼痛及轻度疼痛发生率均高于对照组,重度疼痛发生率低于对照组(P<0.05)。观察组不良反应发生率低于对照组(P<0.05)。观察组对内瘘肢体美观度的总满意率高于对照组(P<0.05)。结论:直行式锐针扣眼穿刺法在透析患者内瘘中的应用效果显著,能够降低患者的疼痛感,提高一次穿刺成功率,降低不良事件的发生,改善患者的内瘘肢体美观度,有较高的应用价值,可在临床推广应用。Objective:To investigate the application effect of straight sharp needle buttonhole puncture in internal fistula of dialysis patients.Method:From May 2020 to May 2021,152 patients receiving maintenance hemodialysis in the outpatient department of Guangdong Agricultural Reclamation Central Hospital were selected as the research subjects.According to the random number table method,they were divided into observation group and control group,76 cases in each group.In the observation group,the puncture of internal fistula was performed by straight sharp needle puncture method,while in the control group,the puncture of internal fistula was performed by traditional sharp needle puncture method.The puncture related indexes,pain degree,adverse reactions and satisfactory appearance of internal fistula limbs were compared between the two groups.Result:The success rate of one-time puncture in the observation group was higher than that in the control group (P<0.05);there were no significant differences in blood flow after puncture,venous pressure and dialysis treatment efficiency (KT/V) between the two groups (P>0.05).The incidences of painless and mild pain in the observation group were higher than those in the control group,while the incidence of severe pain was lower than that in the control group (P<0.05).The incidence of adverse reaction in observation group was lower than that in control group (P<0.05).The total satisfaction rate of internal fistula limb aesthetics in the observation group was higher than that in the control group (P<0.05).Conclusion:The application effect of straight sharp needle buttonhole puncture method in hemodialysis patients with internal fistula is significant.It can reduce the pain of patients,improve the success rate of one-time puncture,reduce the occurrence of adverse events,and improve the aesthetic degree of patients’ internal fistula limbs,which has high application value and can be promoted in clinical application.
