关节镜辅助下Endobutton钢板治疗下胫腓联合分离的初步效果分析

摘　　要：目的:探究关节镜辅助下Endobutton钢板治疗下胫腓联合分离的初步疗效。方法:选择2019年10月-2020年10月东莞市中医院收治的65例下胫腓联合分离患者作为研究对象,按双盲法将其分为对照组(螺钉固定治疗,n=30)和试验组(关节镜辅助下Endobutton钢板治疗,n=35)。对比两组术前、术后2个月后踝关节功能评分[以美国足踝外科协会(AOFAS)踝-后足评分系统评价]与痛觉模拟评分法(VAS)评分、并比较两组术后2个月并发症发生率、治疗优良率。结果:术后2个月,两组踝关节功能评分、VAS评分较术前均改善,且试验组踝关节功能评分高于对照组,而VAS评分低于对照组(P<0.05)。试验组并发症发生率为2.86%,低于对照组的20.00%(P<0.05)。试验组治疗优良率为94.29%,高于对照组的76.67%(P<0.05)。结论:在关节镜辅助下进行Endobutton钢板治疗下胫腓联合分离,能提升临床效果,降低并发症风险,改善患者的踝关节功能,减轻疼痛。Objective:To explore the preliminary efficacy of arthroscopic assisted Endobutton plate in the treatment of distal tibiofibular syndesmosis separation.Method:From October 2019 to October 2020,65 patients with distal tibiofibular syndesmosis separation in Dongguan Hospital of Traditional Chinese Medicine were selected and divided into the control group (screw fixation,n=30) and the experimental group (arthroscopic assisted Endobutton plate,n=35) according to double-blind method.The ankle function scores (evaluated by AOFAS ankle hind foot scoring system) and pain analogue scale (VAS) scores before and 2 months after operation were compared between two groups,the incidence of complications and the excellent and good rate of treatment 2 months after operation were compared between two groups.Result:2 months after operation,the ankle function scores and VAS scores of two groups were improved compared with those before operation,the ankle function score of the experimental group was higher than that of the control group,the VAS score was lower than that of the control group (P<0.05).The incidence of complications in the experimental group was 2.86%,which was lower than 20.00% in the control group (P<0.05).The excellent and good rate of the experimental group was 94.29%,which was higher than 76.67% of the control group (P<0.05).Conclusion:Arthroscopic assisted Endobutton plate in the treatment of distal tibiofibular syndesmosis separation can improve the clinical efficacy,reduce the risk of complications,improve ankle function and relieve pain.
