摘 要:目的:探讨急诊床旁超声在急性呼吸困难快速鉴别诊断的价值。方法:将2020年4-9月江门市新会区人民医院急诊科接收的急性呼吸困难患者72例作为对照组,2020年10月-2021年4月江门市新会区人民医院接收的急性呼吸困难患者79例作为研究组。对照组行常规检查,研究组行急诊床旁超声检查。比较两组检查时间、急性呼吸困难病因诊断准确率及入院后不同时间的氧合指数(OI)。结果:研究组检查时间短于对照组(P<0.05)。两组重症肺炎、肺水肿、肺栓塞、COPD、气胸、胸腔积液各病因诊断准确率及整体诊断准确率比较,差异均无统计学意义(P>0.05)。研究组入院1、2 h的OI均高于对照组(P<0.05)。结论:急诊床旁超声在急性呼吸困难病因鉴别诊断中,可缩短检查耗时,快速查明病因,迅速纠正氧合状况。Objective:To explore the value of emergency bedside ultrasound in the rapid differential diagnosis of acute dyspnea.Method:From April to September 2020,72 patients with acute dyspnea received from the Emergency Department in The People’s Hospital of Xinhui District in Jiangmen were selected as the control group;from October 2020 to April 2021,79 patients with acute dyspnea received from the Emergency Department in the People’s Hospital of Xinhui District in Jiangmen were selected as the study group.The control group were performed routine routine examination,the study group were performed emergency bedside ultrasound examination.The examination time,the diagnostic accuracy of the cause of acute dyspnea and the oxygenation index (OI) at different times after admission were compared between two groups.Result:The examination time of the study group was shorter than that of the control group (P<0.05).There were no statistical differences between two groups in the diagnosis accuracy rate of each cause of severe pneumonia,pulmonary edema,pulmonary embolism,COPD,pneumothorax,and pleural effusion and the overall diagnosis accuracy rate (P>0.05).The OI of the study group at the 1st and 2nd hour after admission were higher than those of the control group (P<0.05).Conclusion:Emergency bedside ultrasound in the differential diagnosis of the cause of acute dyspnea can shorten the examination time,quickly identify the cause,and quickly correct the oxygenation status.
