摘　　要：目的介绍某三甲医院基于信息系统功能对PIVAS细胞毒性药物医嘱进行合理用药前置审核模式的建立,保障患者的用药安全。方法基于细胞毒性药物医嘱错误类型及占比,利用信息系统开发合理用药软件,建立前置审方系统,实现“系统审核+人工审核”的医嘱审核模式。结果优化细胞毒性药物医嘱审核模式后,不合理处方条数由286条降至90条,错误率由1.59%降至0.50%。结论通过信息系统的不断优化以及智能化审方系统的运用,不合理医嘱条目数减少,提高了工作效率,降低了用药差错,有效保证了患者用药安全,是值得考虑推广的细胞毒性药物医嘱审核模式。OBJECTIVE To introduce the establishment of a pre-review model for rational use of PIVAS cytotoxic drugs in the hospital based on the function of the information system to ensure the safety of patients′medication.METHODS Based on the type and proportion of cytotoxic drugs in medical order errors,an information system was used to develop rational drug use software,and a pre-audit system was established to realize the“system audit+manual audit”medical order audit mode.RESULTS After optimizing the cytotoxic drug medical order review mode,the number of unreasonable prescriptions was reduced from 286 to 90,and the error rate was reduced from 1.59%to 0.50%.CONCLUSION Through the continuous optimization of the information system and the use of the intelligent prescription review system,the number of irrational medical order entries has been reduced,work efficiency has been improved,medication errors have been reduced,and patient medication safety has been effectively ensured.It is a cytotoxic drug medical order review that is worth considering for promotion mode.
